1. Purpose

The purpose of this procedure is to outline the responsibilities of SZC and other
personnel under the control of SZC to formally notify the issuance of a SRCC on a
relevant plant system or work area.

2. Scope

This procedure applies to each Section Manager within SZC for their scope of supply
at Tanjung Bin Power Station Site. Sub-contractors shall work to this procedure
under the direction of the involved Section Manager and will be subject to
surveillance and or audit.

3. Associated Procedures

4. Definitions

SRCC: Safety Rules Clearance Certificate, the means by which all parties will be
advised that plant or equipment is to be energized for the first time and from which
point it is mandatory to have access to that plant or equipment using the SZC safety
documentation.

Sub-contractors: Manpower utilised to carry out erection / commissioning work at

the work face.

Sub-sub-contractors: Manpower utilised by a main Erection Sub-contractor to

facilitate the work or part of the work in a specified work face area.

Vendor Representatives: Equipment Suppliers direct manpower utilised to erect or
commission equipment at the work face.

Energised: System or part system that has been made live with electricity, fluid
pressure, air pressure, stored energy (filling), hazard substances and any means,
which may have the potential to harm.

5. Responsibilities

The responsibilities within this procedure are connected totally with the safe working
of personnel within or adjacent to a specified work face, plant system, or area where
specified equipment is to be energised and therefore has the potential to harm.

The responsibility is directly with the SZC Site Manager and partner Site Managers
to ensure that all potential affected parties are formally notified of the SRCC
procedure and of specific SRCCs that have been issued. This responsibility is then
passed to the Sub-contractor management to ensure that all manpower at or near
the specified work face is aware of the potential hazard and that a SRCC has been
issued.

For personnel who are not considered in a supervisory role, they will not receive
SRCCs, therefore training is required for the various recipients of the SRCC in terms
of what is required to be done upon receipt. Such personnel shall treat all installed
plant/apparatus as in service and under the Safety Rules unless the individual has
received confirmation from the responsible party.

6. Procedure

Distribution of the SRCC will be carried-out by the Commissioning Manager / Field

Operation Leader nominee as follows:

a) Attach the Signature Proforma (See Appendix C) to the front of the SRCC;

b) Identify from the SRCC the work unit/location of the plant/apparatus and
compare with the standard distribution from the Matrix Distribution Sheet
(See Appendix B);

c) Mark onto the Signature Proforma in the tick-box provided the required
distribution;

d) Note the current date, time and SRCC number in the SZC SRCC File
Register;

e) Pass the SRCC and Signature Proforma to the SZC Site Manager or in his
absence the Commissioning Manager to verify the distribution;

f) Upon return of the SRCC and Signature Proforma from the SZC Site
Manager or Field Operation Leader with the confirmed distribution, copy the
SRCC document into the required number of copies;

g) Issue a copy of the SRCC to each Superintendent in accordance with the

distribution requirements and also circulate the Signature Proforma to all
recipients for acknowledgement by signature, date and time;

h) The Signature Proforma must be signed by all no later than 1700 hours of the
same working day (SRCCs received on Saturday shall be received by 1700
hours on Monday.) If the Signature has not been returned, identify its
location, retrieve and obtain outstanding signature;

i) Enter into SZC File Register the date and time received;

j) On a weekly basis, the SZC File Register must be cross checked with the
filed Signature Proformas to ensure good record keeping is being
maintained; (Partners Quality Manager will audit every month)

k) All distribution recipients must ensure that the SRCC is distributed onwards in
hardcopy format to immediate subordinates, sub-contractors, vendor
representatives and other appropriate persons working in or near the
specified work face.
