Guidelines for Offline Nominations to the Sectoral Engagement (SE) Grants

Pursuant to CMO No. 48, s. of 2016, which repealed CMO No. 14, s. 2016, the following
document outlines some reminders and notes regarding the nomination process for the Sectoral
Engagement Grants under the CHED K to 12 Transition Program.

General Documents:

Notes:
1. All guidelines regarding eligibility, and processes are detailed in the CMO No. 48, s.
2016

2. The following documents are required of all nominees:


a. Re-entry Action Plan Templates (Appendix 5)
b. SHEI Nomination Form (Appendix 6) *To be jointly accomplished by the grantee and SHEI
c. Grantee Curriculum Vitae Templates (Appendix 7)
d. DBP Application Requirements
i. Application Form for DBP Prepaid Card
ii. 1x1 ID Picture
iii. Photocopy of Any Valid ID
e. Medical Certificate (issued within the last six months)
f. Proof of Citizenship (NSO authenticated birth certificate, Information page of valid
passport, or voters ID)
g. The following are required of HEI-Initiated Partnerships:

3. For Sectoral Engagement Grants with EXISTING HEI initiated partnerships:
a. HEI-Initiated Partnership Form
b. Engagement Proposal Form
c. Partner Profile Form
d. Certified True Copy of Notarized MOA between the HEI and Partner (if existing);
OR
e. Exchange of letters or written certification of the verbal agreement of the
partnership, duly signed by representatives of the HEI and the partner (if newly
initiated or prospective)

4. The following requirements must be accomplished by the SHEI:


a. SHEI Nomination Form (refer to number 3) * To be jointly accomplished by the grantee
and SHEI
b. Collation of all individual requirements into an IRSE Nomination Packet

5. All forms must be accomplished digitally.

6. Ensure that all contact numbers and email addresses of faculty and non-teaching staff
nominees are functioning email addresses.

7. Regarding submission:
a. Submit the SHEI Nomination Form via email (WORD FORMAT)
b. Submit hard copies of all requirements to :
