ALL REQUIRED DOCUMENTS IN SECTION 5 BELOW, UNLESS STATED
OTHERWISE, ARE REQUIRED. INCOMPLETE OR LATE APPLICATION WILL
NOT BE PROCESSED. YOUR COOPERATION IS HIGHLY APPRECIATED.

5.0 CHECKLIST ON REQUIRED DOCUMENTS


1 Authorization letter signed by the Owner if the application form is submitted by
the Owners Representative.
2 Approved drawings and specifications of the repair / renovation works from
Architect and Professional Engineer.
3 Letter of indemnity from the Owner indemnifying the Management against all
claims arising out the repair / renovation works (Please use the attached
template).
4 Original copy of insurance cover note (Contractor All Risks Policy) stating the
following (note: this requirement only applies to renovation work costing
MYR20,000 or more):
a) Incorporate the following Parties forming part of the Joint Insured(s):
Urusharta Cemerlang Sdn Bhd as the Landlord and Isonex Sdn Bhd as
Property Manager.
b) Limit of Indemnity for Third Party Liability not less than
i) MYR1,000,000 for contract value of between MYR20,000 to
MYR50,000, or
ii) MYR3,000,000 for contract value of between MYR50,001 to
MYR100,000, or
iii) MYR5,000,000 for contract value of above MYR100,001, in any one
accident and Unlimited during the period of insurance.

c) Include the following Extended Terms / Clauses:
i) The Landlord and the Management shall not in any way be liable
howsoever to the Owner or to any of the Owners Authorized
Persons or to others who may be permitted to enter or use the
Unit(s), the Pavilion Residences, the Land or any part thereof
(including the renovation period, if any) for any happenings
accidents death or injuries howsoever sustained by any of the
aforesaid persons or any other person or for any loss or damage
howsoever caused to any properties, merchandise, furniture,
fittings or equipment whatsoever in the Unit(s) or any other part of
the Pavilion Residences or the Land, arising from the acts or
omissions, whether negligently, carelessly, intentionally or by way
of misconduct, default or otherwise, of the Landlord and/or the
Management or any of their respective Employees, Servants,
Agents, Workmen, Contractors of Pavilion Residences or the
Land.
ii) Loss or damage to the Pavilion Residences properties caused by
or arising from Parties / Contractors is covered up to the loss
amount.

5 Refundable security deposit of MYR8,000.00 by way of checks made payable to


the Management, i.e. ISONEX SDN BHD.
6 Non-refundable administrative charge of MYR100.00 made payable to the
Management, i.e. ISONEX SDN BHD.
7 If it is a major repair / renovation works, or if the works involve hacking or
alteration to the existing structures, a non-refundable design review fee of
MYR1,000.00 made payable to the Management, i.e. ISONEX SDN BHD.
