This resource will assist teachers with communicating with students and families, gather
feedback for reflecting and self evaluating instructional practices from students and families point of view
of the culminating school year.

End of the Year Exit Ticket Assessment

Purpose: To help teachers gain valuable feedback from


students and families by reflecting on the school year.
While at the same time, allowing students to reflect upon
their own learning and academic take-aways from the
school year, as well as a way for parents to gauge their
involvement in their childs learning.

Connections to Framework: Using this resource will allow


teachers to communicate with families and solicit and
utilize the families feedback to support student learning.
This resource will help teachers successfully engage
families as partners in the instructional program in the
classroom.

How to use: Below are examples of a blank student exit


ticket and a family exit survey, as well as a completed one
of each.

Next steps: After students have completed their exit ticket,


teacher and student could conference about the students
perspective vs. the teachers perspective about the student
and their learning. Once all the family surveys are
collected, teachers can then reflect on their practice and
how it incorporates the students families feedback on
their childs school experience. This will help the teacher
become distinguished in Domain 4 in the next school year,
by utilizing the feedback to plan assignments and projects
deemed successful and to build off of those to create even
more great projects and assignments in the future.
