AFFIDAVIT OF DISCREPANCY

I, MAE PIMENTEL MONTENEGRO, of legal age, with residence and postal address at
Purok Maliwanag, Barangay Sumo-sumo, Tago, Surigao del Sur, after having been duly sworn to under
oath in accordance with law hereby depose and state:

That when I took the Civil Service Examination in Davao City on July 3, 2003, I entered in the
form provided to us the entry of my birthdate as May 1, 1982;

That the said birthdate was the date that I knew as my date of birth since I was a child;

That, however, I discovered lately that the date of my birthday, as provided for in my
Certificate of Live Birth is April 30, 1982;

That from the moment I learned of my true birthdate, I used it in providing information to all
my undertakings, such as providing informations in my Personal Data Sheet;

That, as such, there is a discrepancy in my birthdate in the record of the master list in the Civil
Service Commission and that of my Personal Data Sheet;

That I execute this Affidavit to attest the fact of the abovementioned reason and the veracity of
the above facts and for whatever legal purposes it may serve best

IN WITNESS WHEROF, I have hereunto affixed my signature this 8th day of July, 2015 at
Tandag City, Surigao del Sur.
