    Unit No:-4 I TPS:- Parli-V Unit Capacity:- 210 MW


    1. HO Code Station Time:- 15:25 Date:- No of days from last
    Code: - LBIO2-120L5 svnc. :- 41 davs
    2.Operating condition at the time of Event:-
    Load Coal cycle in service I Oil support
    MW=02 NIL cD-L,2,3 ,4
    3. Nature of Event: - Forced withdrawl.

    4.Name of protection (first up)and other Main Protections operated:-


    1) Turbine tripped 2) Boiler tripped 3) Generator main breaker tripped. 4) Master trip Relay 86G
    & 86TT operated.

    5. Observations:-
    Turbine bearing No-6 vibration were on higher side.(H=ttO/22,V=100/15,A=t30123 ).
    Hence unit withdrawn for tightening wedges.

    Shift In charge

    6. Remedial Action/ Work done:- 1.TG Bearing No.6 Wedges tightened. Additional one
    wedge fitted.

    7. Root Cause Analysis:- Turbine bearing No-6 vibration are on higher side due to gap
    between packer plate & LP cylinder foot.

    B. Preventive Action (ShortTerm):- Additional Tapered wedges to be placed between
    packer plate & LP cylinder rear foundation frame. Monitor the position of tapered
    wedges regularly .Monitor vibration of Bearing No.5,6,7 regularly.

    9. Preventive Action (Long Term):- Permanent solution in consultation with OEM is to
    be sought. Tender for rectifying vibration problem during AOH, is in process.
