1. The enclosed Allied Medical Publication AMedP-4.8 Edition A, Version 1,


PRE- AND POST- DEPLOYMENT HEALTH ASSESSMENT, which has been
approved by the nations in the MCMEDSB, is promulgated herewith. The agreement
of nations to use this publication is recorded in STANAG 2235.

2. AMedP-4.8, Edition A, Version 1, is effective upon receipt and supersedes


AMedP-39, Edition A, which shall be destroyed in accordance with the local
procedure for the destruction of documents.

3. No part of this publication may be reproduced, stored in a retrieval system,


used commercially, adapted, or transmitted in any form or by any means, electronic,
mechanical, photo-copying, recording or otherwise, without the prior permission of
the publisher. With the exception of commercial sales, this does not apply to
member nations and Partnership for Peace countries, or NATO commands and
bodies.

4. This publication shall be handled in accordance with C-M(2002)60.

USA a. While the STANAG clearly specifies that its requirements are
minimum essential information requirements, the STANAG does
not fully address information requirements which through USA
experience are critical to adequate pre-and post-deployment
health surveillance.

b. All of the listed pre-and post-deployment minimum essential


data points are collected for USA military forces but not at the
same time or using a single form. Therefore, while Department of
Defense Form 2795, Pre-Deployment Health Assessment, and
Department of Defense Form 2796, Post-Deployment Health
Assessment, are used to collect some of the minimum essential
data, these forms in adddition to multiple data collection and
management systems collect and retain the information for
retrieval, reference and analysis as necessary.

c. One drawback of the system is that it seems to imply a fairly


static deployment. The system needs to be able to track where
service members go during their deployment within a theater of
operations, so that health episodes can be more discreetly aligned
with potential environmentally or operationallty related incidents.

CHAPTER 1

INTRODUCTION

0101 GENERAL

The importance of good health among soldiers in military operations is vital for a
successful result on the battlefield. Healthcare as part of the preparations in advance
of the deployment, during the operations and after the redeployment is in this respect
vital.

0102 AIM

The aim of this document is to ensure that personnel participating in NATO


operations receive pre- and post-deployment health assessments, and that these
assessments include at least a minimum set of data elements.

0103 AGREEMENT

1. Participating nations agree to collect at least the following information on all


deployed forces:

a. The minimal essential information elements for pre-deployment health


assessments which are listed in chapter 2.

b. The minimal essential information elements for post-deployed health


assessments which are listed in chapter 3.

2. To minimize health problems during a NATO mission, a pre-deployment health


assessment of deploying personnel is essential. After a mission it is important to
apply a post-deployment health assessment for identifying health problems and/or
exposures which could potentially result in post-deployment health problems. The
pre - and post - deployment health assessments are intended to supplement more
comprehensive longitudinal health assessments conducted periodically during a
military members career.

CHAPTER 2

PRE-DEPLOYMENT HEALTH ASSESSMENTS MINIMUM ESSENTIAL


INFORMATION

0201 GENERAL

The pre-deployment health assessment is vital, in order to make it possible to give


the single soldier a satisfactory health care during her/his time of deployment.

0202 DEMOGRAPHICS

The following list gives as a minimum a reliable and exact information about where
the single soldier is going to be deployed, the name and nationality of the assessing
unit and an exact information about whom the soldier is:

a. Location of deployment

b. Country of deployment

c. Name of the military operation

d. Date of assessment

e. Unit

f. Nation

g. Name

h. Date of Birth

i. Rank

j. Gender

k. Service (e.g. Army/Navy/Air Force/Marines)

l. Component (e.g. Active/Reserve)

0203 HEALTH ASSESSMENT

The following Health Assessment information is, as a minimum, necessary to collect


before the single soldier will be deployed:

a. Do you have any medical problems?

b. Do you have undergone any surgical procedures within the last six months?

c. Do you have had any significant changes in health since last examination?

d. Do you have any dental problems?

e. When did you last see a dental officer?

f. Are you on any restricted duties because of medical / dental problems?

g. Do you have any allergies?

h. Are you pregnant?

i. Are you currently taking any prescription medicines?

j. If yes on the question above, which ones?

k. Do you have sufficient supply?

l. Do you have the required number of prescription glasses (if worn) for use
during deployment?

m. Do you have sought counselling or care for your mental health?

n. Do you have any health concerns or questions regarding this deployment?

0204 HEALTH ASSESSMENT BY MEDICAL PERSONNEL

The following important information should be collected and assessed by authorized


medical personnel:

a. Health assessment.

b. Is there cause for referral?

c. Was any medical briefing provided?

d. Are immunizations completed?

0205 DISPOSITION

The information gathered above, should make basis for answering the question
below and give the necessary comments:

a. Does the person fulfil the necessary requirements for being deployed?

POST-DEPLOYMENT HEALTH ASSESSMENTS MINIMUM ESSENTIAL


INFORMATIONS

0301 GENERAL

After a mission it is important to apply a post-deployment health assessment for


identifying health problems and/or exposures which could potentially result in post-
deployment health problems. The Post-Deployment Health Assessment will form the
basis for giving the deployed soldier the necessary health support after having been
deployed.

0302 DEMOGRAPHICS

The following demographic information is important:

a. Location of deployment

b. Country of deployment

c. Name of the military operation

d. Date of assessment

e. Unit

f. Nation

g. Name

h. Date of Birth

i. Rank

j. Gender

k. Service (e.g. Army/Navy/Air Force/Marines)

l. Component (e.g. Active/Reserve)

0303 HEALTH ASSESSMENT

The following information should be collected by medical personnel:

a. Did you have any medical / dental problems which developed during this
deployment?

b. Are you on any restricted duty because of medical / dental


problems?

c. Have you sought, or do you intend to seek, counselling or care for your
mental health as a result of this deployment?

d. Do you have any health concerns or questions regarding possible


exposures or events during this deployment?

0304 HEALTH ASSESSMENT BY MEDICAL PERSONNEL

Based on the information collected above, the medical personnel will make the health
assessment as listed below, as a minimum:

a. Health assessment

b. Exposure concerns

c. Is there cause for referral / follow-up?

d. Was medical threat debriefing provided?
