Re : Notice to Explain

This is in connection with the charge leveled against you for your

You are hereby directed to explain in writing within five (5) days
from receipt hereof why you should not be subjected to disciplinary action,
which may include termination, for your commission of the foregoing act.

Likewise, to afford you further opportunity to explain, you are


hereby required to appear at an administrative hearing to be conducted on
__________________ at ________ a.m/p.m. held at the office address
located ________________________, where you may bring along your
representative or counsel if you so desire, in order for you to further clarify
your side of the above-stated charge.

Please note that unless satisfactorily explained, the charge cited

Regulations)______________, which would justify the Company in
imposing disciplinary action against you or terminating your employment

Failure to respond to this Memorandum shall be construed as a
waiver on your part to be heard on the charge imputed against you and
the Company shall proceed to decide on your case on the basis of
documents and evidence at hand.

Finally, considering the nature of the charge leveled against you and
that your continued presence at the Company premises pending the
administrative hearing thereof may pose as a threat to the property of the
Company, you are hereby placed under preventive suspension for thirty
(30) days effective immediately upon receipt of this Memorandum.
