Functional Behavioral Assessment

Description of Student: William (DOB: 1/22/2001) is a 14 year old Hispanic male currently residing in
Meriden, Connecticut with his biological mother, Elizabeth Rodriguez.

William began attending the Raymond Hill School on 7/11/2012 due to negative behaviors impacting his
ability to function in a school environment. William has a history of physical and verbal aggressive
behaviors. William was expelled from school in June 2012 and was suspended a total of 28 days for
mostly fighting on the bus with peers. Currently, William has difficulty using appropriate language,
separating himself from peers and using positive coping strategies on a consistent basis. He often leaves
the classroom without permission and engages in explosive behaviors when facing a negative emotion.
William is designated as needing special education services under the primary disability of emotional
disturbance.

William previously resided in an adoptive home, though after AWOLing from this home he was placed in
a group home. William transitioned to his biological mothers care in September 2014. He was receiving
intensive in home therapy during the first five months of residing with mother. William has not been
receiving consistent outpatient therapy since being discharged from his in home services. William is
currently prescribed

Description of site: Raymond Hill School, part of Klingberg Family Centers, is a clinical day school in
which students are labeled as needing special education services due to behavioral and emotional
issues. William is in a seventh grade classroom with eight students in the class, four intensive staff a
teaching assistant, and the special education teacher. William also receives the support of an intensive
staff member in the afternoons

Definition of Behavior:

After reviewing much of the School-wide Information System data (SWIS), Harmony reports, Incentive
Chart, and Scatter Plot, William most frequently demonstrates disruptive and out of bounds behaviors.
Disruptive behaviors include inappropriate language and defiance of rules. He enters a classroom while
swearing loudly, in order to gain the attention of his peers. When having a conversation with adults, even
when at baseline, William uses profanity frequently. In addition, William often is unable to separate
himself from peers in the classroom, and engages in disruptive behaviors with others. He engages in side
conversations, ignores staff redirections and has the ability to encourage others to remain off task in the
classroom.

Out of bounds is defined as leaving his assigned area or room without permission or according to his
schedule. When out of bounds, William often joins peers who are also off task or influences peers to join
him. William will leave the school building and campus at times. If he leaves the campus, he is
considered AWOL and staff follow the AWOL policy, which includes notifying the New Britain Police
Department.

At times, William is able to demonstrate the ability to remain in the classroom with minimal issue. He
can accept feedback from staff and focus on his assignments. William demonstrates minimal insight into
how his behavior is drastically different at times and is unaware of the factors that impact his ability to
maintain his negative emotions and participate in the classroom.

Data Collection:

During this academic school year, staff has collected data utilizing SWIS. SWIS data is comprised of
disciplinary forms that are filled out by staff involved in major incidents or after 3 redirections have been
given per PBIS procedure. In addition, data was collected from a scatter plot from March 31-May 8, 2015.

In class data collection:

This is data collected by the teacher or classroom staff. Data was not collected when Will was in crisis or
out of bounds.

On 3/31 Will used inappropriate language no fewer than 47 times and had 15 negative interactions with
peers. Will was OOB for 37 minutes.

0n 4/1 Will used inappropriate language no fewer than 58 times and had 8 negative interactions with
peers.

On 4/2 Will used inappropriate language no fewer than 8 times and had no negative interactions with
peers.

On 4/6 Will used inappropriate language no fewer than 30 times and had 1 negative interaction with a
peer.

peers. Will was OOB for 1hour and 58 minutes.

peers. He was also OOB for over an hour.

peers.

On 4/26 Will used inappropriate language no fewer than 57 times and had 3 negative interactions with
peers. He was OOB for 30 minutes.

On 4/27 Will used inappropriate language no fewer than 163 times and had 17 negative interactions
with peers. Will went OOB.

On 4/28 Will used inappropriate language 259 times and had 4 negative interactions with peers.

On 4/29 Will used inappropriate language 64 times and had 1 negative interaction with peers. He went
AWOL with said peer.

On 4/30 Will used inappropriate language 93 times and had no negative interactions with peers.

On 5/6 Will was OOB for 1hour and 45 minutes.

On 5/7 Will was OOB for 1 hour and 55 minutes.

On 5/12 Will was suspended out of school for assaulting a staff member.

Key:

OOB is out of bounds it means leaving the classroom without permission but not leaving the grounds.

Inappropriate language does not include minor curse words such as Hell, damn, sh*t.

AWOL is absent without leave, meaning he left the grounds.

Harmony Reports:

According to the Congregate Care Incidents Report, William has been in seclusion twice for attempting to
harm a peer and not agreeing to be wanded prior to entering the school building. He was restrained
once after engaging in a physical altercation with a peer. William was in school suspended for a total of
six days due to becoming physically aggressive with peers, destroying property, pulling the fire alarm and
AWOLing.

William would benefit from continued intensive staff services


A Behavior Intervention Plan (BIP) should be in place in order to assist William
throughout the day.
