The undersigned petitioner, ____________, unto this Honorable Court

respectfully states that:

1. He is a Filipino citizen, single, twenty seven (27) years old and a

A, A-1 and A-2;

4. He was admitted to the Philippine Bar on May 24, 2017, a copy of

the certificate of admission is hereto attached as Exhibit B;

5. He is a bona fide member in good standing of the Integrated Bar of

the Philippines, ____________Chapter, attached is a copy of his
Certificate of Good Standing as Exhibit C, and has paid his
membership due on May 17, 2017 as evidenced by IBP Official
Receipt No. ____________ issued by the Integrated Bar of he
Philippines, hereto attached are copies as Exhibits D and D-1;

6. He has paid his Professional Tax for the fiscal year 2017 under
Professional Tax Receipt No. ____________, a copy is hereto
attached as Exhibit E;
 7. He has not been charged nor convicted of any crime involving
moral turpitude and as such is allowed to practice his profession
and hold the office of Notary Public.

8. He possesses all the qualifications and none of the disqualifications

of a Notary Public;

9. He has not applied for commission before any other jurisdiction;

10.He undertakes to inform the Executive Judge within ten (10) days
in case of any change in the given address and to submit his
monthly report religiously;

11.He now desires to be granted Notarial Commission as Notary

Public for and in the City of ____________.
