PETITION
PETITIONER, and unto this Honorable Court, most respectfully avers as
follows:

1. He is of legal age, married, Filipino, and a resident of Apsayan, Gerona,

Tarlac;

2. He is a bona fide member of the Philippine Bar.

3. His Law Office is located at Carriedo St., Pob. Sur, Paniqui, Tarlac.

4. He passed the 2015 Bar Examinations, admitted to the Bar last June
20, 2016 and was assigned the Roll Number 65627 (Annex A);

5. He is a lifetime member of the Integrated Bar of the Philippines with

Life Member Roll No. 015231 as manifested in the attached certification ( Annex
B);

6. He is a bona fide member of the Integrated Bar of the Philippines,

Tarlac Chapter as per attached certification (Annex C);

7. He has not been convicted or accused of any felony involving moral

turpitude and has all the qualifications and none of the disqualifications of a
Notary Public;

8. He has secured an NBI Clearance hereby attached and marked as

Annex D;

9. He has paid his current Professional Tax, as per Xerox copy of OR No.
4441498, dated June 22, 2016 which are hereto attached and marked as
Annex E;

10. A photocopy of the picture of his office is hereto attached as Annex F;

11. He hereby attaches a photocopy of his specimen signature marked as

Annex G;
 12. He desires to be appointed as Notary Public for and within the Province
of Tarlac for a period of two (2) years effective upon the date of the Order of this
Honorable Court, ending December 31, 2018;

WHEREFORE, it is most respectfully prayed of this Honorable Court that

herein petitioner be appointed as Notary Public for and within the Province of
Tarlac for a period of two (2) years effective on the date of the Order of this
Honorable Court and ending on December 31, 2018.
