AFFIDAVIT OF DISCREPANCY

I, GENALYN B. MARTINEZ, of legal age, single, citizen of the Republic of the

Philippines and presently residing at Sto. Nio, Apurawan, Aborlan, Palawan,
after having been duly sworn to in accordance with law, do hereby depose
and say:

1. That my name is MARILYN BONIFACIO MARTINEZ, as stated and registered

in my birth certificate issued by the National Statistics Office, Republic of
the Philippines;

2. That during my childhood and adult years, I have been accustomed to

the use of the name GENALYN BONIFACIO MARTINEZ;

3. That I assumed the name GENALYN B. MARTINEZ, when I signed official

and other kinds of documents;

4. That the names MARILYN B. MARTINEZ and GENALYN B. MARTINEZ refer to

one and the same person; and

5. That I am executing this Affidavit to clear any discrepancy which may

arise from official documents bearing either name MARILYN or
GENALYN;

IN WITNESS WHEREOF, I have hereunto set my hand this 24th day of April,
