Contract Review on the

Project of Classrooms in Lugdungan ES and Dicayas ES under CY 2016 Basic

Educational Facilities Fund (BEFF) Batch 1 CLUSTER 2

Observation/s Noted:

a. Perusal of contract submitted revealed that the contractor submitted two (2) different sets
of Audited Financial Statements wherein both were duly received by the BIR.

b. Post-Qualification Evaluation Report is dated on March 11, 2016. However, Notice of

Post-Qualification is dated on March 10, 2016. As such, Notice of Post-Qualification
was already given prior to Post-Qualification Evaluation Report is completed. Thus,
request is hereby made to seek for clarification on the said issue.

c. The attached photocopy of PRC Licensed of Project Manager and Project Engineer was
expired. As noted, the expiration date of the PRC Licensed of Project Manager and
Project Engineer is on December 3, 2013 and September 23, 2014, respectively.

d. Item V of Program of Works which provide a corresponding Tax Computation of 5% of

(Item III+IV) amounting to P15,523.41. However, per recomputation it does not
reconcile with the said amount.

Lacking of Supporting Documents as provided under Item 3.1.2 and Annex A of COA
Circular 2009-001 dated February 12, 2009 and Annex A IRR of RA 9184:

a. No Certificate of Availability of Fund as Certified by the Division Accountant; and
