My name is Destu. I am a nurse student from Notokusumo Nurse Academy.

Now, i
am working at Makmur Hospital. i work in morning shift start from 8 am until 2 pm. I arrived
15 minutes before starting work. I always prepare to care for patients before 8 am. i start to
work at 8 am. First, i look for patients and check their blood pressure. Sometimes, we change
patients bed linen if look dirty. After that, i prepare medication for patient, giving oral drugs
and injection. if it is done, I have free time to assess the patient's health problems. i have
choosen a patient, his name is Mr. Dimas. He suffered a fracture on his leg.

Nurse : good morning ?

Patient : morning.

Nurse : previously, let me introduce my self. My name is destu, i am nurse who duty from 8
am to 2 pm, here I will ask some questions to maximize the nursing process to you, in
advance if you are willing ?

Patient : of course

Nurse : whats your name sir ?

Patient : my name is dimas

Nurse : how are you today sir ?

Patient : oh, i am still not very good

Nurse : what do you feel ?

Patient : i feel pain in my right leg

Nurse : Do you know what is causing the pain ?

Patient : of course I know this pain because my right leg was broken

Nurse : pain like what you feel ?

Patient : like a stabbed

Nurse : can you tell me what can cause the pain ?

Patient : if i move my right leg, i will feel the pain

Nurse : if there is a scale of 1 to 10, with 1 being the least pain and 10 being the worst pain,
how would your rate your pain ?
Patient : maybe 7

Nurse : really ? it must have been very ill

Patient : Sometimes I bite the pillow to stifle the pain

Nurse : sir, maybe you can try relaxation with deep breathing, can you ?

Patient : i will try, can i ask a medication for reduce my pain

Nurse : wait a moment sir, i will consult with the senior nurse or the doctor. If you some
help you can push the emergency bottom at your bedside. Do you have questions ?

Patient : no, thank you

Nurse : okay sir, i will go to nurse station and i will come back immediately

After that, I consulted with a senior nurse, if given permission to administer the
medicine, I will give it, but if not I will go back to the patient and say while not allowed.
After finish, I write the data in the medical record with the new care plan for the patient. I and
senior nurses have always shared about the progress or condition of the patient before we
finished our shift, I ready go home at 2 pm
