Observations and Recommendations:

The entire site was visited and there were eight buildings under construction in
varying stages. They are 1.Administrative building, 2.Penicillin building, 3.Utility &
Engineering building, 4.Contraceptive building, 4.IV fluid building, 6.QC, PD &
Tablet RM, PM&FG building, 7.Ware House and 8.Gate House building. Ware
house building was mainly observed.

Ware House building

Observations:

a) Slab reinforcement placement was going on.

b) Extension of tie bars was less than 3 in most cases.

c) Column tie bars were not bent to 135 degree in most cases.

d) In scaffoldings, cross bracing was missing in some places.

e) Some Column main bar extension was kept for further construction.

f) Lapping at beam column joint which is prohibited as per Code.

g) Reinforcing bars in a few columns were found placed outside the tie bar.

h) Some support of the scaffolding was misplaced.

l) In some columns, tie bars were found not properly tied with main bars.

j) In many locations it was found that first tie was not placed 2 from
 support.

Recommendations:

a) Concrete clear cover shall be strictly maintained.

b) Alternate tie hooks should be strictly maintained in the columns.

c) Provision to keep minimum 3 inch extension of tie bars should be

ensured.

d) Clear cover at outer face of beam column must be maintained by using

concrete blocks.

e) Lapping of bars at beam column joint shall be avoided.

f) Column tie bars at beam-column joint must be placed as per drawing.

h) Adequate numbers of orthogonal cross bracings must be provided in the

scaffolding system.

i) Support of scaffolding should be placed on compacted ground.

j) Column tie bars shall be tied securely with the main rebars.

k) First stirrup of beams shall be 2 from support.

l) Adequate monitoring and preventive measures shall be taken against any

movement/ displacement/ settlement of scaffolding system during

concrete casting.

Fig. Some bars placed outside the tie, which should be within tie.

Fig. Anchor length of tie bar is less than 6db or 3 inch

Fig. Some of the hooks are not properly tied with main bar & most of them

are less than 135 degree.
