assessor-collector's office. Replacement fee is $6.00, plus $0.50 automation fee. Do not mail cash.

replacement for cosmetic or readability reasons, and that any current license plate and/or registration sticker in my possession
that is being replaced have been returned to the County Tax Assessor-Collector's office.

I also state that the replacement plate(s) and/or sticker being issued will not be used or displayed on any vehicle other than the
above described vehicle, and that if the license plate(s) and/or sticker being replaced are recovered, they will not be used or
displayed on any vehicle.
