Humana Associate Benefit Summary

Full-Time Associates (30 or more hours per week)

Medical Benefits – Eligible immediately upon completion of MyReality Check. Choose from 4 High
Deductible Health Plans by using Humana’s online Enrollment Center (including the Rx Calculator). Bi-
weekly costs range from $26.00 to $56.92 for single coverage and $38.46 to $134.36 for family coverage
levels.

HDHP plans are HSA-compatible and Humana matches contributions to an associate’s HSA account
based on salary.

Non-tobacco user discount is available.

Coverage is available for Extended Family Members, including domestic partners.

Spousal surcharge applies if covering a spouse/domestic partner who has eligibility for coverage through
another employer.

Dental Benefits — Eligible immediately. Choose from 3 plans. Biweekly costs range from $.75 to $4.22
for single coverage and $4.75 to $22.44 for family coverage. Coverage is available for Extended Family
Members, including domestic partners.

Vision—Eligible immediately. Includes benefits for eye exam, glasses, and contact lenses and
discounts on LASIK/PRK services. Biweekly costs range from $2.51 to $7.49. Coverage is available for
Extended Family Members, including domestic partners.

Holidays – Eligible Immediately. Associates receive 7 paid holidays + 2 personal holidays. Standard
holidays include: New Year’s Day, Memorial Day, July 4, Labor Day, Thanksgiving (2 days), Christmas
Day. (1 personal day first year if hired on or after July 1.)

Vacation – Associates earn vacation on a bi-weekly basis, based on length of service and scheduled
work hours. Vacation time can be taken after 3 months of employment.

Bi-weekly amounts earned for associates scheduled to work 40 hours per week:
0-9 years completed: 4.62 hours (120 hours annually)
9+ years completed: 6.15 hours (160 hours annually)

Full-time associates scheduled to work 30-39 hours per week earn prorated vacation based on their
scheduled hours. For example, a new associate scheduled to work 30 hours per week would earn 3.46
vacation hours bi-weekly (90 hours annually).

Short-Term Income Protection For Illness (STIP) – Eligible immediately. Associates earn .384 days
bi-weekly, based on their scheduled work hours.

Short-Term Disability (STD) – Eligible immediately. Benefits start after 5 consecutive days of absence
from work. Pays 66 2/3 percent of base pay for up to 6 months.

Long-Term Disability (LTD) – Eligible immediately. Benefits start after 6 months of absence due to
illness or injury. Pays up to 66 2/3 percent of base pay. Pre-existing condition exclusions apply.

Full-Time Associates (30 or more hours per week)

Flexible Spending Accounts (FSAs) – Eligible immediately. You may contribute up to $5,000 a year
on a pretax basis to either or both accounts to pay eligible health care expenses not covered by
insurance (Healthcare FSA) or eligible child or adult day care expenses (Dependent Care FSA).

Basic Life Insurance – Eligible immediately. Options include basic term life coverage of 2 times salary
or $50,000 if your annual salary is over $25,000. AD&D benefit equals additional 1 times life benefit in
case of accidental death or bodily injury. Business Travel Accident coverage of 3 times salary. No cost
to associates.

Voluntary Term Life – Eligible immediately. You may purchase additional coverage up to 6 times
salary. Lesser amounts can also be purchased for spouse or domestic partner and children. You must
elect coverage for yourself in order to cover spouse or domestic partner and children.

Long-Term Care Insurance – Eligible immediately. Options include monthly benefit of $2,000 or
$4,000. You may enroll yourself, spouse or domestic partner, adult children, siblings, parents (and in-
laws), and grandparents (and in-laws). Family members must be between the ages of 18 and 80. You
do not need to elect coverage for yourself in order for family members to enroll.

Retirement & Savings Plan – Savings Account (401(k)): Eligible immediately. Automatic enrollment
of 4% pre-tax on your first paycheck and Automatic Savings Increase applied each January—up to 6%
pre-tax. You may contribute up to 35 percent of pay on a pretax basis, subject to the annual IRS dollar
limit, and up to 2 percent of pay on an after-tax basis. Participants over 50 may also elect an additional
“catch up contribution.” Choice of investment options. Immediate company match of 100% of the first 1%
pre-tax contribution, plus 50% of the next 5% pre-tax contributions. Full vesting after 2 years of service.
Rollovers allowed. Note: May opt-out of Automatic enrollment and/or change the Automatic Savings
Increase percentage.
Retirement Account: Eligible after 2 years of service. Fully funded by the company. Annual
contributions of 4 percent of total eligible compensation plus 4 percent of any eligible compensation over
the Social Security taxable wage base. Choice of investment options. Associates must be employed on
12/31 of each year in order to receive the Retirement contribution. Associates are always 100% vested
in their Retirement Account.

Educational Assistance – Tuition assistance for associates scheduled to work 40 hours per week for
approved courses related to your role and overall company objectives. Courses must start after your
employment date. Pays 100 percent of first $5,000 in tuition and course-specific fees. Tuition Assistance
for associates scheduled to work 30-39 hours per week, program pays 50 percent of the first $2,500 in
tuition and course-specific fees. Scholarship program through the Humana Foundation awards up to
75 scholarships of $2,500 to children of Humana associates with three years of service.

Adoption Assistance – Eligible after one year of service. Reimburses 80 percent of eligible expenses
up to $1,200 for the legal adoption of a child.
