Introduction including overview of area inspected and activities taking place:

This is the report of the safety inspection carried out by me at Al Taweelah Alumina Area
490. The total work area of inspection is 15000sq.m and is located on Shaheen road at a
distance of about 1 km from ATA area 490 around. The inspection was carried out on 12 th
August 2017 at 13:00, the manpower during peak time as per man power histogram more than
550. Apart from the employees visitors comprising of vendors, subcontractors with employees
and legal enforcement employees were also present. The workforce mainly from various parts of
India such as Bihar, Orissa and West Bengal who can speak Hindi was supervised by the main
contractor staff.

The project work at the inspection site was undertaken by Al Taweelah Alumina. The purpose
of this inspection was to identify the good practices and areas of opportunities and to bring them
to managements attention. All appropriate hazards and recommendations have been mentioned
in subsequent pages enabling management team to work on legal obligations as per various
laws like Health and Safety act 1974, OSHA etc.

The inspection was aimed at identifying the uncontrolled hazards and its consequences
associated with the activities in the construction work site. The main construction work carried
out in the time of work place inspection Civil Construction activities such as; excavation,
backfilling, earthwork, concrete works etc. Mechanical, Electrical, maintenance works, shuttering
carpentry and scaffolding activities mainly working at height activities etc. Crane movements
such as JCB operation, rigging, lifting and material transfer etc.

Executive Summary:

While there are certain good practices which are being observed and management must be
congratulated on meeting their obligations as required by various laws and regulations, few areas
of opportunities need immediate attention. Working on these important findings will ensure that
the company is compliant with rules and regulations and also will emphasize the commitment of
management towards a safe work culture thus boosting employee morale. The issues found
during the course of inspection are violations of various sections of Indian Factories Act & ILO
conventions. Control Measures have been recommended for the hazards identified. Main issues
found during the course of inspection

1- Poor segregation of the construction site including unprotected excavations from the
pedestrians and vehicular ways. This can lead to accidents by collision of vehicles with pillars,
scaffolds, machineries etc. in the construction site. Pedestrians and vehicles can also fall into the
unprotected excavations.
2- Oxygen and LPG cylinders are not segregated in the worksite can lead to fire and explosion
hazards in case of any leaks in the cylinders and also by collision with vehicles due to poor
barricading. Worker carrying out welding without covering the operation using
Fire blanket can also possess fire, explosion & burn hazard.

3- Working at height in scaffolding platform without handrails and safety harness and use of
unsafe access equipment like scaffolds and ladders. This can lead to injury or death of workers
by falling from height or by collapse of access equipments.

4- Poor welfare facilities for the workers such as lack of rest rooms, eating rooms, pure drinking
water, adequate number of latrines & urinals, washing facilities, poor first aid facilities etc. This
can lead to poor worker morale, motivation, attitude and personal hygiene.

5- Mobile cranes operating in the proximity of live overhead power lines can cause electric
hazards such as electric shock, burn and fire.

Main findings of the inspection:

1. Segregation of construction site from general public:

It is found that segregation of pillar construction site from vehicular roadways and
pedestrian walkways is inadequate. As the construction work is carried out at the middle of
vehicular roadways, pedestrians and passengers in the vehicles using the roadways are at high
risk than the workers because they are unaware of the hazards and risks arising from the
construction activities and due to absence of PPEs. In many places fencing is found
discontinuous and in some places barricading tapes and nets are used for fencing instead of
metallic fences. Even though speed limit boards and warning signs are displayed close to the
fencing in the construction site, most of them are damaged or hidden from the public. As a result
drivers become unaware of the speed limits to be followed and can collide with the pillars,
scaffoldings, machineries, materials, workers in the construction site and lead to injuries or death
of drivers, passengers and workers in the construction site. An excavation in the construction site
where no work is carried out is found uncovered and barricading is done using barricading tapes.
Also there was no warning signs indicating excavation work and lighting was inadequate in the
vicinity of the excavation. This can cause drivers and pedestrians become unaware of the
excavations close to the roadways and pedestrians and vehicles can fall into the unprotected
excavations leading to injury or death. Solutions include provide adequate segregation of
construction site from general public using metallic fences, provide adequate speed limit boards
and warning signs including excavation work clearly visible to the public, ensure adequate
lighting levels in the excavation site and protecting uncovered excavations. Benefits to the
employer by implementing these control measures include avoidance of costs associated with
accidents to general public, compensation claims, improved reputation of the organization etc.
Costs to the organization for replacing barricade tapes & nets with metallic fences, displaying
speed limit boards & warning signs and providing adequate lighting is around 15,000 AED.

2. Fire & Explosion Hazards:

It is found that Oxygen & LPG cylinders of gas cutting assembly are stored together & left
unattended in the pillar reinforcement construction area near the vehicular roadways. The area
where the cylinders were stored was segregated using nets instead of metallic fences. Vehicles
passing through the roadways can collide with the cylinders due to poor barricade leading to fire
& explosion hazard. Leak in the cylinders an also lead to fire & explosion hazards. A worker is
found carrying out welding operation on the top of pillar cap reinforcement without covering the
area using fire blankets. The sparks passing through the gaps in the reinforcement can also
cause fire, explosion hazards & burns to the people passing through the roadways below.
Solutions include storing oxygen & LPG cylinders in separate storage booths at least 5m away,
providing barricade of the construction site using metallic fences, displaying warning signs
indicating fire & explosion hazards, covering the welding area using fire blankets. Benefits to the
employer by implementing these control measures include avoidance of costs associated with
fire & explosion accidents such as medical treatments for burn injuries, property damage,
compensation claims by employees & public, legal costs, fines from regulatory authorities &
improved reputation by reducing the accident rates. Costs to the organization associated with
fixing metallic fences, displaying warning boards, providing cylinder storage booths & providing
fire blankets is around 10,000 AED. Responsible persons for implementing these control
measures include employer, directors & senior managers, safety officers, supervisors & workers.
3. Working at height:

Worker is found not wearing safety harness and without handrails while working at the
top of the scaffolding platform in the construction site at the time of inspection. The worker was
manually handling a metallic sheet on the top of girder. Worker can fall from height due to slips,
trips on the working platform or due to carelessness leading to injuries or death. It has been also
found that workers using unsafe access equipments like ladders and scaffoldings for access to
the pillars. Both ladder and scaffolding were not inspected, materials got deteriorated and rusted.
Scaffolding platform was not boarded fully and toe boards and proper guard rails were not
provided. Ladder was resting on unlevelled ground without any supports. Workers can fall from
access equipment at height by slip or trip on the working platform of scaffolding or on the rungs
of the ladder. Worker can also get injured by collapse of ladders or scaffoldings due to
deterioration or by falling of ladders due to absence of supports. Solutions include providing
information, instruction & supervision of workers in the use of safety harness, maintaining good
housekeeping in the work platform to prevent trip hazard, using slip resistant soles to prevent slip
hazards, regular inspection of access equipments for damage, deterioration etc. Benefits to the
employer by implementing this control measures include avoidance of costs associated with
death, injury or ill health of employees, sick pay, fines from regulatory authorities for poor health
& safety standards, delay of work etc. Costs to the organization for implementing the above
control measure is around 6,000 AED.

4. Welfare Facilities:

During the course of inspection it is found that welfare facilities for the workers were
inadequate. Rest rooms or eating rooms were not provided for the workers near the worksite. It
has been found that workers were eating in open area in the contaminated atmosphere near the
worksite. Drinking water cans were exposed to the contaminated work environment. Adequate
number of urinals, latrines & washing facilities were also not provided near the worksite. First aid
facilities including first aider were not adequate for the number of persons employed. Poor
welfare facilities can reduce workers morale, motivation & attitude towards the work.
Contaminated foods & drinking water can cause ill health effects. Poor sanitation & washing
facilities can affect the personal hygiene of employees. Solutions include providing rest rooms,
eating rooms, washing facilities at a safe place near the worksite, ensure adequate supply of
pure drinking water away from the contaminated work environment, provide adequate number of
urinals & latrines near the worksite, sufficient number of first aid facilities with employment of first
aiders etc. Benefits of the employer by implementing these control measures include improved
workers morale, motivation & attitude leading to improved productivity, avoidance of fines from
regulatory authorities for poor health & safety standards, reducing the consequences of the injury
by providing first aid to the victim etc. Costs to the organization for providing adequate welfare
facilities are around 10,000 AED.

5. Electric Hazards:

Mobile crane in the construction site is found operating close to the proximity of overhead
power lines without isolating the supply through it. The crane was lifting a pillar reinforcement in
the pillar construction site. Accidental contact of structural members of crane with live overhead
power lines can cause electric shock, burns or fire hazards to the operator and people in the
vicinity. Solutions include erection of goal post barriers under the overhead power lines to
indicate safe clearance distances for operating the mobile cranes, isolating the power supply
through the overhead power lines when operating close to it, clearly marking the danger zones
with warning signs on each side of the power lines, employment of banks man or marshals while
operating near overhead power lines etc. Benefits to the employer by implementing these control
measures include avoidance of costs associated with medical treatment for electric shock, burns,
property damage due to fire, compensation claims, and improved corporate image by reduction
in accident rates. Cost of erecting goal post barriers and displaying warning signs is around
5,000 AED.
Conclusion

A number of good practices were found during the course of inspection such as display of up
to date health & safety Policy, up to date inspection & labelling of fire extinguishers etc. However
a number of issues were found during the inspection .The main issues found were work at
height, poor segregation of construction site from public, electric hazards, poor welfare facilities
and fire & explosion hazards.

Most of these recommendations can be implemented at low cost, time & effort to the
organization. Implementation of these recommendations can bring economic benefits to the
organization by avoidance of costs associated with accidents such as medical treatments, sick
pay, compensation claims, production delays, fines from regulating authorities, legal costs etc.
By cultivating a positive health & safety culture in the organization, workers morale, motivation &
attitude towards the work can be improved leading to increased productivity. Other benefits to the
organization include improved reputation, image etc.
