American Association of Colleges of Nursing (AACN) Essential VII of The Essentials of

Baccalaureate Education for Professional Nursing Practice (2008) is Clinical Prevention and

Population Health. Nurses play an essential role in overall community health by promoting

healthy practices and encouraging prevention efforts. To be successful in the community setting,

nurses must be able to work with all members of the population as well as be aware of how each

member of the community interacts with the surrounding environment.

Exemplar

To illustrate Essential VII, I chose the Community Health Assessment completed in my

Population-Focused Nursing Practice/Practicum course. Essential VII recognizes that in

population-focused nursing, the aggregate, community, or population is the unit of care (2008).

As a baccalaureate graduate, I will need to be able to identify ways to determine the health of a

community, plan education and prevention strategies, and work in collaboration with other

healthcare professionals to ensure the best possible outcomes are reached.

Reflection

The Community Health Assessment assignment was a major project for my population-

focused class. After choosing a small community within my local area, I completed a community

profile inventory, a community needs assessment using a windshield survey, and then analyzed

that data to identify the top health problems in that community. Then, I formulated goals and

objectives for each of the health problems and made recommendations based on the Healthy

People 2020 Initiatives. This project allowed me to examine health promotion and disease

prevention at the individual and population level (AACN, 2008), which is the main focus of

Essential VII. Finding ways to improve the health of a given population is a vital component of

baccalaureate nursing. The culmination of my project was an audiovisual presentation

summarizing my findings.
