JOINT AFFIDAVIT
OF TWO DISINTERESTED PERSONS

We, ALFREDO C. EUGENIO, of legal age, Filipino citizen, and a

resident of Barangay Sta. Maria, Camiling, Tarlac, Philippines, and
SOFINA A. CALI, of legal age, Filipino citizen, and a resident of
Barangay Manaquem, Camiling, Tarlac, Philippines, respectively, after
having been duly sworn to in accordance with law, do hereby depose and
state THAT:

1. We both personally know DAISY ANN BURBOS

ESTEBAN, (Daisy Ann, for brevity), born on December 15, 1980
to Spouses Osmundo D. Esteban and Fausta C. Burbos in
Camiling, Tarlac, Philippines;

2. Daisy Ann became known as MRS. DAISY ANN

ESTEBAN OBISPO after she got married to Joel Sison Obispo;

3. Lately, while Daisy Ann was organizing her personal

effects, she came over the birth records of her children and after
scrutinizing them, she discovered that in the Certificate of Live
Birth (attached as Annex A) of her daughter, Dawn Marie
Nicole Esteban Obispo (Dawn Marie Nicole, for brevity), her
Maiden Surname was recorded as ESTABAN, instead of
ESTEBAN as recorded in her Certificates of Live Birth and
Marriage (attached as Annexes B and C, respectively). Hence,
the discrepancy;

4. We are not related to Daisy Ann by affinity or

consanguinity except that of being her friends and
acquaintances;

5. We are executing this Affidavit to attest to the fact that

the names DAISY ANN BURBOS ESTEBAN, as recorded in her
Certificate of Live Birth, DAISY ANN B. ESTEBAN, as recorded
in her Certificate of Marriage, DAISY ANN BURBOS ESTABAN,
appearing in the Certificate of Live Birth of Dawn Marie Nicole
Esteban Obispo as her mother, and DAISY ANN ESTEBAN
OBISPO, refers to one and the same persons, and that is the
mother of Dawn Marie Nicole Esteban Obispo, our friend and
acquaintance;
 6. Finally, we are executing this Affidavit to attest to the
truth of the foregoing statements and for whatever legal intents
and purposes it may serve.
