Your household must make less than twice the guidelines based on your household size (count all the people living at your residence. Under certain circumstances, roommates may not be considered as part of the same household.

A Medical Clinic for Uninsured Patients

PURPOSE The purpose of the Garden City Community Clinic is to provide safetynet health services and dental care to those who are economically disadvantaged and without health insurance in the Treasure Valley. CLINIC SERVICES

FINANCIAL ELIGIBILITY Each patient is screened for eligibility at the clinic. Patients must meet established income guidelines (see back of brochure.) The following is an overview of those who typically qualify: Those who are unemployed Those who work part time Those working without insurance coverage Those with seasonal or low paying jobs Those attempting to become selfsufficient while leaving public assistance Those who are homeless or residing in shelters

Medical Chronic care (i.e. diabetes, hypertension, asthma, etc.) Short-term, non-emergency illnesses Medication Assistance (some fees apply) Social services and referrals to other agencies Counseling Dental Dental screening and hygiene Fillings, root canals, and extractions The Clinic DOES NOT provide Casting or cast removal Narcotics, controlled drugs and weight loss drugs Filling prescriptions from outside providers Prenatal care

APPOINTMENT PROCEDURES
Primary Care Medical Services Patients can call 384-5200 during the week for an appointment. Availability varies depending on volunteer physicians schedules. Walk-in appointments are not made. Follow-up medical exams are made by appointment as the schedule allows. Basic Dental Services Dental services are only provided upon a physicians referral for established medical patients of the following clinics: Garden City, Vineyard, Friendship and Family Medicine Health Center.

THOSE WHO DO NOT QUALIFY Those who have government sponsored coverage: Medicaid, Medicare, VA benefits Those who have group or individual medical/dental insurance If you suffer from work-related injuries involving Workers Compensation If you are uninsured but receive an adequate annual income to cover medical expenses IT IS IMPORTANT THAT YOU BRING THE FOLLOWING TO YOUR FIRST VISIT: Current picture identification Social security number Verification of household income (pay stubs, bank statement, W-2 form, etc.) Any medical history paperwork

We appreciate the gift made by volunteer health care and other professionals. Please understand our limits in services we are able to provide. Cash donations by patients are accepted but not required for service.
