AFFIDAVIT
(RE: HISTORY OF EMPLOYMENT)

I, ROMEO R. CECILIA, married to Ms. Dinnah S. Cecilia, of legal age and with postal
address at Blk. 7, Lot 44, Jaguar St., Villa Luisa, Dasmarias City, Cavite, after being duly
sworn to in accordance with law, depose and state: That - -

1. I was employed in the following companies;


a. Jade Valley Restaurant as a Busboy from October 1981 to November 1982;
b. Miles Express Cargo Forwarders as Clerk from December 1982 to November 1984;
c. Anito Group of Company as a Customer Specialist from December 1984 to July
1995;
d. Espiritu Gun Store as a Liaison Officer from August 1995 to September 1997;
e. Touchstar Motors Corporation as Operations Officer from December 2005 to
November 2007;

2. The said companies have denied my persistent request for a Certificate of Employment,
Employment Contract and Certified Job Description, I have discharged during my employment
thereat due to the following reasons:

a. Jade Valley Restaurant, Miles Express Cargo Forwarders and Espiritu Gun Store
had ceased their operations and the said companies did not provide me any of these
documents;
b. Anito Group of Companies had already merged to other companies and I have no
idea where it is located so I could not provide a Certified Job Description which
contains the duties and responsibilities I have discharged during my employment;
c. Touchstar Motors Corporation and Anito Group of Companies did not issue any
Employment Contract because it is not stipulated in the companies rules and
regulations;
d. Touchstar Motors Corporation cannot provide a Certified Job Description because
the companies had already ceased their operations;

3. In lieu of which, what I can only submit as substitute document to prove my previous
affiliations with the companies are these documents appended below:

application for permanent residence in Canada and to attest to the truth and veracity of the
foregoing facts and circumstances for whatever legal purpose it may serve best.
