Our client MARK LESTER LOZARES seek our legal services to ask for you to
refund from your company the initial premium he paid from availing expanded core-10
Health care program because of the very late processing and suspicious program of
your company.

With this letter therefore, a formal and final demand is hereby made against you
to settle the above-mentioned matter within five (5) days from receipt hereof. Failure to
heed this demand much to our regret, we shall have no other recourse but to institute
appropriate legal actions against you in court which shall entail payment of twenty
(20%) percent attorneys fees and other consequential damages.
