AFFIDAVIT

I,ALBERTA WAGA-LIMSON, Filipino, of legal age, and

resident of # 222 Narra St. Fatima I-K, City of Dasmarinas, Cavite,
after being duly sworn to in accordance with law, depose and state:
That - -

1. I am the mother of the late NELBERT GALAROSA WAGA

and her late wife MARIA FE SUASON CERILLA , who died
intestate on June23,1996 and May 6,1997 at Dasmarias,
Cavite;

2. My late son NELBERT G. WAGA left my grand son JARED

LORENZO CERILLA WAGA who was born on August 1, 1995
at City of Dasmarinas, Cavite ,

3. Unfortunately through inadvertence the parents of JARED

LORENZO CERILLA WAGA failed to registered because of
the above-mentioned fact with the local Civil Registrar of City
of Dasmarinas.;

4. Through this affidavit I hereby declare under oath that

JARED LORENZO CERILLA WAGA was born on August 1,
1995 in city of Dasmarinas, Cavite and failure to immediately
caused the registration of the above-mention birth with the
Civil Registrar was duly on account of the above stated reason;

5. I am executing this affidavit to attest to the truth and veracity

of the foregoing statements and for whatever legal purpose
the same may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature
