SUPERANNUATION GUARANTEE AUDIT QUESTIONNAIRE

I refer to your previous correspondence letter dated 3 May 2017. You requested we complete the Status
of the worker questionnaire to establish the facts of the working relationship between Rebecca Jones
and Elite Building Services Pty Ltd (the taxpayer) for the following periods:

1 October 2015 to 31 December 2015

1 January 2016 to 31 March 2016
1 April 2016 to 30 June 2016

The taxpayers bookkeeper has completed the Status of the worker questionnaire regarding Rebecca
Jones for the periods stated above. Please refer to the attached document.

Please find attached copies of tax invoices submitted by Rebecca Jones to the taxpayer for her hours
worked in the business. The invoices represent submissions over a three-month period.

We also note the following points:

1. A search on the ABN lookup on Rebeccass ABN (ABN: 56 800 900 700) suggests she obtained
her ABN immediately before commencing her contract with the taxpayer. We understand
Rebecca undertook registration on her own behalf.

2. When Rebeccas contract of engagement changed to formally being employed by the taxpayer,
her rate and pay was reduced to factor in the taxpayers obligations for employee taxes, i.e.
superannuation guarantee, and responsibilities to Rebecca as an employee. The taxpayer has
advised us that Rebecca understood this.

3. Points 1 and 2 above provide evidence that Rebecca understood the nature of the arrangement
between herself and the taxpayer.
