Testimony of the petitioner Juan Santos De la Cruz III is being offered to serve as his
direct testimony in the Petition and to prove the:

1. All the allegations in the Petition including all annexes appended thereto;
2. All other matters, facts and circumstances relevant and material to this Petition.

III. JUDICIAL AFFIDAVIT PROPER

1. Q: Do you undertake to answer the questions to be asked of you, fully conscious

that you will do so under oath, and that you may face criminal liability for
false testimony or perjury?

A: Yes.

2. Q: Please state your name, age, residence, and occupation.

A: I am JUAN SANTOS DELA CRUZ III, 45 years old, single, residing at B27 L5
Potterridge St., South Forbes, Makati City, Philippines, and an Architect.

3. Q: Why are you here now?

A: To give a sworn statement by way of judicial affidavit, the same to constitute

as my direct testimony in the above-captioned petition for adoption.

4. Q: Are you the same JUAN SANTOS DELA CRUZ III, the Petitioner in this Petition
for Adoption of PATRICIA YSABEL LAUREL?

A: Yes.

5. Q: For the record, please state the name and address of the Lawyer who is now
conducting or supervising your examination and place where the examination
is being held now?

A: Atty. Olivia G. Pope is conducting/supervising my examination now at her

Law office (Pearsons, Specter, Pope Law Offices) located at Unit 27, Zuellig
St., Magallanes Drive, Makati City.

6. Q: In what language do you want your examination to be conducted?

A: In English.
7. Q: Do you know PATRICIA YSABEL LAUREL?

A: Yes.

8. Q: Can you state her residence at preseent?

A: B27 L5 Potterridge St., South Forbes, Makati City, Philippines.

9. Q: How did you know her?

A: I was her legal guardian from 1999 to 2015.

10. Q: What was the circumstance that lead to your appointment as her legal
Guardian?

A: The death of the parents of Patricia, due to a plane crash in Macau sometime
in 1998.

11. Q: Why was the guardianship terminated in 2015?

A: It was terminated because Patricia had reached the age of majority on the 21st
day of March 2015.

12. Q: How old is Patricia now?

A: 20 years old.

13. Q: So would you agree, if I say that there is a 25-year difference between your
age and that of Patricia?

A: Since I am now 45, and she is 20 years old, I agree.

14. Q: Is Patricia currently studying?

A: Yes. She is a 3rd year Culinary Arts student in De La Salle University, Taft Ave.,
Manila.

15. Q: Who pays for her tuition and other expenses?

A: I personally pay for all her expenses.

16. Q: Are there no properties left by the parents to support her needs and expenses?
 A: All real and personal properties by the late Sps. Laurel had been sold during
the guardianship; the proceeds were utilized to provide all the needs and
expenses of Patricia for the period of 16 years. A portion thereof were utilized
to satisfy the debts of the spouses.

17. Q: Do you have any documents to support your statements?

A: Yes. A fateful copy of the Order to Sell issued by the Family Court of Makati,
Deed of Absolute Sale of the real and personal properties sold, Receipts for the
debts paid including a Clearance of my financial accountabilities issued by the
Bureau of Internal Revenue, are hereto attached as Annex A B D and F.

18. Q; You mentioned earlier that you are an Architect, correct?

A: Yes.

19. Q: Can you tell this court pertinent matters about your job?

A: I am a Senior Architect at BuildIt Architecture and Design Studio, located

at 27 Metrosquare Bldg., 377 Ortigas Avenue, Greenhills, San Juan, Metro
Manila. I am an employee thereto since August 2000; and my average monthly
salary is Php 50,000.00, or Php 600,000.00 per year. I worked for 40 to 50
hours from Mondays to Fridays.

20. Q: Have you ever been convicted of a crime? Or Is there any pending case in a
Court, Tribunal or Agency against you?

A: No, I have not been convicted of any crime, and to the best of my knowledge,
there has been no pending case against me.

21. Q: What is/are your purpose/s in filing this Petition for Adoption?

A: My objective is to give Patricia a legitimate status, enabling her to possess all

the rights accorded to a legitimate child. Also, to make her a part of a new
family that will provide her with love, care, understanding and security
towards the full and harmonious development of her personality. Lastly, to
secure that all financial needs necessary for her growth and education, be
promptly accorded.

22. Q: Pursuant thereto, have you undergone any counseling sessions?

A: Yes. I have completed the counseling services as required by the Department
of Social Welfare and Development.
23. Q: What is your Prayer to this Court?

A: That this Petition for the Adoption of PATRICIA YSABEL LAUREL be granted;
and a Decree of Adoption be issued.

24. Q: What Documents do you wish to submit to this Court?

A:

1. As Exhs. A to A-1, i.e., Order to Sell the (1) A parcel of land covered by TCT
No. T-123456, located in B7 L15 Escala St., Cambridge Villages, Makati City,
Metro Manila, Philippines; and (2) a condominium unit covered by CCT No.
789101, located at Rm 727, 7th Fl., Pacific Towers Bldg., Amorsolo St., Makati
City;

2. As Exh. B i.e., Order to Sell a Toyota Altis, 2011 Model, covered under
Certificate of Registration No. 23456, Official Receipt No. 3456789 issued by
the Land Transportation Office, Makati City;

3. As Exhs. C to C-2 i.e., (1) Deed of Absolute Sale of Real Propert covered by
TCT No. 123456; (2) Deed of Absolute Sale of Real Property covered by CCT
No. 789101; and (3) Deed of Absolute Sale of Motor Vehicle as aboved-
described;

4. As Exhs. D to D-3 i.e. (1) Receipt No. 45678 executed by Dr. Sansa Stark;
(2) Receipt No. 56789 executed by Dr. Theon Greyjoy; (3) Receipt No. 67890
Engr. Arya Tyrell; and (4) Receipt No. 89101 Atty. Cersei Lannister.

5. As Exh. E i.e., Clearance of Financial Accountabilities with respect to the

termination of Guardianship, issued by Bureau of Internal Revenue of Makati;

6. As Exhs. F to F-2 i.e., (1) Certificate of Employment in BuildIt Architecture

and Design Studio; (2) Income Tax Return from 2000 to 2016 issued by the
Bureau of Internal Revenue; and (3) Working Attendance Record.

7. As Exhb. G i.e., School records of Patria Ysabel Laurel issued by De La Salle

University Manila from A.Y. 2014 to 2017;

8. As Exhb. H i.e., NBI Clearance, issued by National Bureau of Investigation; and
