RE: Authority to Pick Up NBI Clearance

TO WHOM IT MAY CONCERN,

I am RUTH MAUREEN C. RILLON and I applied for an NBI clearance and

was scheduled last September 6, 2017 to appear at your satellite office inside
Robinsons Galleria. My name got a hit and was advised to come back to
your office today, September 15, 2017. Due to my unavailability to appear
personally today, I hereby authorize my father, EDWIN A. RILLON, to pick
up my NBI Clearance from your office.

2. My original SSS ID for authentication
3. Photocopy of my SSS ID
4. Photocopy of Edwin Rillons government issued ID
