Pre-game Warm-up & Intermission Timing Report

The goal of automating this form is to help speed up the process for Off-ice officials as
well as help Dave and Lisa have an easy way to receive and extract data. With the help of
SurveyGizmo, we were able to automize this form. The form can be filled out on a desktop,
mobile device, or even a tablet. The form will adapt to the size of the screen of the device and
make it easily visible for the user. A few of the features include page logic (OIOs will only see
information that they need to see, no need to fill out irrelevant information), auto suggesting of
names (textboxes will fill with players names that you wish to enter), and a Save & Continue
button at the top of each page (giving the user the option to enter his or her email and a URL of
the form will be sent to their email directly where they can continue where they left off). These
features, plus many more, will give us at the NHL the ability to help streamline this process as
well as many more soon.
How to log on:
LINK Site/VO2:
The user will be able get to the form by going through the new LINK site (which will be
operational soon-pictured below). He or she will need to go to the Hockey tab (at the top of the
screen-labeled by the blue arrow in the picture below) and then click hockey operations.
Then the user will click on the OIO Homepage section (labeled by the red arrow in the picture
below). This OIO homepage will be a new feature for Off-ice officials, getting to see the newest
updates and policy changes happening in the National Hockey League. Once on the OIO
homepage, the user will be able to quickly access Venue Operations 2 (labeled where the URL of
the form will be accessible (the location of the URL has not been decided yet but will be clearly
labeled for the Off-ice Officials).

Once on the OIO homepage, the user will be able to quickly access Venue Operations 2 (labeled
by the red arrow in the picture below). In VO2 the URL of the form will be accessible (the
location of the URL has not been decided yet but will be clearly labeled for the Off-ice
Officials).
SurveyGizmo:
The user can also sign in through Surveygizmo. He or she will have their email registered in the
SG database and will be able to access the form by logging into SurveyGizmo. The user will
only be able to fill out that form and not being able to alter or access any other forms under the
NHL account.
General Information:
This first part of the form is to gather general information about the game as well as what took
place (violations). There are the common questions of what teams were playing, the date of the
game, and game number. However, there is also the first question of form which is, Was there a
Violation?. This will determine which part of the form the OIO will see/need to fill out. With
this page logic in place the user wont have to fill out any unnecessary information. There are
two pictures below, one being the look of the form before any information is filled out and
another picture right before the user will go to the next page.

Q: Was there a Violation?


- This question has three options (Pre-game Warm-up, Intermission, or none).
- This question type (checkbox) gives the user the option to select as many of the choices
as they want depending on their observations of the game (which is labeled by the first
blue arrow on the page).
- The choices the user makes on this question will determine which part of the form he or
she will see (Example: If the user selects just Intermission, he or she will go directly to
the Intermission page after filling out the rest of the General Information).
Q: Game Number:
- This question is a simple textbox. It requires a numeric answer and is required for OIOs
to fill out when completing the form.
Q: Date (labeled by the 2nd blue arrow in the picture above):
- This question has a calendar icon next to the textbox. By clicking on the calendar icon,
the user will be able to select from the restricted dates (Example: the day of and the day
after a game will only be selectable).
- This feature will require OIOs to fill out the form within 24 hours of the game.

Q: Visiting Team/Home Team (labeled by the green arrow in the picture above):
- This is a simple dropdown question. By clicking on the bottom facing arrow the OIO will
be given all 31 options of NHL teams.
- We have populated these options with the NHL teams and the user will only have to
select each team once in the whole form.
Pre- Game Warm-Up:
If the OIO/user picked Pre-game Warm-Up in the first question of the form, this will be the
first page that appears after the General Information page. This section has two total pages with
different features to help expedite the process for the OIOs. As mentioned in the overview, the
Save & Continue button (labeled by the green arrow in the picture below) is available at the top
of the page giving the user the ability to stop where he or she is in the form and save it for later
in the game.
Q: Start of Warm-up, Time on Clock:
- This question appears twice (one for each team-labeled by the red arrow in the picture
below). Notice how each team is clearly labeled above the question (the teams name
populated to this question from the results of the first page).
- There are two textboxes with labels over each one. If there is a violation in the beginning
of warm-ups, the user will enter the minutes in the first text box and the seconds in the
second text box (labeled by the blue arrow below).

- This question has three components with different features attached to each one. The
three sub questions within this page are Players Number, Players Name, and Watch
Time. These questions appear for each team (which is populated into this page from the
first page).
Q: The Players number
- This question is a textbox but must be a number answer (if you enter anything besides a
number, you wont be able to move on).
Q: The Players Name (labeled by the 2nd red arrow in the picture below)
- This question is also a simple textbox; however, it has an auto suggest feature that was
mentioned earlier. By populating all the names of NHL players from this past season into
SurveyGizmo, the user will be able to start typing out a players name and then SG will
suggest the name it believes the OIO is entering. For example, if you start typing Sidney
Crosby, the name should suggest underneath the textbox while typing it out and the user
should be able to select the suggested name. These names are just suggestions so if there
is a call-up and their name might not be in the list of suggested names, the user can still
enter that full name without restriction.

Q: The Watch Time


- This Question is a simple textbox where the OIO will simply enter the time on the clock
of when the violation took place.
*Add Another Feature* (Labeled by the black arrow in the picture below)
- If the OIO wishes to enter another players violation, all they will need to do is select the
button Add Another underneath the previous three sub questions. This feature will
allow the user to add as many player violations as they want. The same three sub
questions will appear again underneath the previously filled out violation. The OIO will
have the flexibility to add as many violations as they need by the simple click of a button.

Intermission Timing:
The Intermission Timing page is comprised of two total pages, each page for an intermission. If
the user selected Intermission as the violation in the very first question of the form and no
other option, this would be the first page the OIO would see. The idea of separating each
intermission into its own individual page is to allow the user to use the Save & Continue
button after the first page of Intermission Timing. If the user chooses to fill out this form during
the game, he or she will have the ability to go page by page and know where they left off.
Q: 1st Intermission Violation Time:
- This question has two textboxes with one box being labeled minutes and the other
being labeled seconds. The user will be able to enter the numeric values into each
textbox (labeled by the blue arrow in the picture below).
- The save and continue button at the top of the page will allow the user to fill out the 1st
intermission page if they want with the option of going to the second page after the 2nd
intermission happens.
Q: 2nd Intermission Violation Time (labeled by the first black arrow in the picture below):
- This question has two textboxes with one box being labeled minutes and the other
being labeled seconds. The user will be able to enter the numeric values into each
textbox.
- The save and continue button (labeled by the second black arrow in the picture below) at
the top of the page will allow the user to go back to this page after the 2nd intermission (if
they choose to fill out this form during the game).
Verification:
This verification section is a wrap up of the form, collecting the users credentials/information.
The name has auto suggested features (the names being suggested-I entered the Crew
Supervisors names), their title (also auto suggested with specific OIO positions), and a signature
that will complete the verification process. If the user selected none on the very first question
of the form, this would be the first page that the OIO would see.

Q: Name (first black arrow on the picture above)


- The name question is a textbox with auto-suggested names encrypted in the question. The
names of the Crew Supervisors have been entered but a full list of OIOs could be
populated into this question.
Q: Title (second black arrow on the picture above)
- The title question is also a textbox with auto-suggested positions encrypted into the
question.
Q: Signature (third black arrow on the picture above)
- The Signature Box gives the user the ability to sign their name (can use your finger/ or
mouse depending on the device you are filling the form on). It is then followed by a
textbox underneath verifying the name of the signee.
Q: Comments/Observations
- Finally, there is an Any Comments/Observations question giving the OIO the ability to
enter any information that was not covered throughout the form.

Email:
An email of the results will then be sent to Lisa Spinelli. She will receive an email containing all
the answered questions in the body of the email as well as receive a PDF of the entire form. Lisa
will also have the ability to go on Survey Gizmo to extract any data that she wants, whether that
is individual responses or a summary of all the responses (example: mid-season summary
report).
