INTRODUCTION

Background of the study

Sto.Nio Municipality health Center started its operation of giving health services

to the citizens of Sto.Nio on May 3, 1980 at the Lapu-Lapu Village, Sto.Nio South

Cotabato. In 2001, under the governance of Mayor Ervin Luntao, it was relocated

beside the Lapu-Lapu proper in Sto.Nio South Cotabato.

The new structure of SNMHC was bigger and more convenient for the patients.

There were additional Services offered to satisfy the citizens, but due to the large

population of the Municipality of Sto.Nio and the province of South Cotabato,

Sometimes the administrator found hard to keep and record all files in a folder. It was

time consuming and the record had a tendency to get lost. The researchers decided to

conduct a study which on Information System with Data Interactive Representation of

Sto.Nio Municipality Health Center.

This study aimed to help the Health Center in computerizing the processes done

in the Heatlh Center like keeping the records of the patients. This study helped the

Health Center record room assignment, provide doctors information and schedule,

update and search the record of the patients, generate individual record of the patients

and the list of registered patients.

Objectives of the Study

General objectives

The study generally aimed to developed and evaluate the Information System
with data Interactive Representation of Sto.Nio Municipality health Center.
Specific Objectives

The study specifically aimed to:

1. Design a record keeping system in:
1.1 Patients reservation and check-up schedules
1.2 List of program and clients such as:
1.2.1 Client health profile
1.2.2 Infection disease prevention
1.2.3 Family health program
1.2.4 Vitamin A supplement for children under five years of age
2. Design a billing system for the Sto.Nio Municipality Health Center
3. Design an inventory system of the Medical supplies
4. Generate reports such as:
4.1 List of patient with corresponding findings
4.2 Individual patient check-up schedule
4.3 List of Admitted outpatient and Beneficiary
4.4 Health program
4.5 Official Receipt
4.6 Stock in and Stock out Medical supplies
5. Provide data Interactive Graphical Representation such as:
5.1 Most arises disease in the Sto.Nio Municipality Health Center
5.2 Number of Client per Month/Year
5.3 Comparison of the Income per Month
6. Evaluation the system in terms of:

Significance of the Study

The study on the development and evaluation of Information System wiData

Interactive Representation of Sto.Nio Municipality Health Center would be beneficial to

the following:

To the Doctors. The study would help the doctors in scheduling their schedule.

To the Personnel-in-charge. The study would help them organize the patients record

and helped them make the processes to be faster, more accurate and more reliable.
To the Patients. The study would help the patients process their records and information

faster and less being consumed.

To the Researchers. This study would help the researchers learn more and get more

ideas for their further study and for the improvement of the conducted study as well.

To the Visitors. The study would help the visitors process to find their patients room

easily.

To the Future Researchers. The study would help the future researchers in relation to

the topic this served as a resference.

Scope and Limitation

The proposed study entitled Information System with Data Interactive

Representation of Sto.Nio Municipality Health Center a system which would record

patients information based on accomplished form, provide a system that can bill patient

charges transaction, provide a system that can manage doctors information and

schedule, and generate the update record of the patients, Patient payment receipt and

doctors information and schedules. The system aimed to convert the present manual

system of the Sto.Nio Municipality Health Center into a computerized one, for the

benefit of the personnel-in-charge and the patients as well. The system could also be

accessed by the authorized personnel with their valid username and password.

Operational Definition of Terms

The following terms were defined theoretically and operationally used in this

study.
 Client Server - describes the relationship between two computer programs in which on

program, the client, makes a service request from another program the server which

fulfills the request.

Computerizing is the process of doing a task which involves computer and other

applicants.

Patient is a person receiving or registered to receive medical treatment.

Doctors Schedule the scheduling system of the doctors recorded in the system.

Effectiveness refers to the effectiveness of the system as function.
