RANK JUSTIFICATION
1 Diarrhoea is an actual problem that
must be addressed first since
according to Hendersons Nursing
Need Theory, an individual achieves
wholeness by maintaining
physiological and emotional balance
and the physiological components
include eat and drink adequately
together with elimination of body
wastes. The problem is prioritized
first because due to progress of the
dilemma it may lead to onset of
other problems/ condition which may
worsen the health status of the
patient.
2 Imbalanced Nutrition is an actual
problem. It is not presently existing
however it may happen if there is the
occurrence of Diarrhoea. According
to Maslows Hierarchy of Needs,
biological and physiologic needs are
on the 1st level on the pyramid. It is
the second prioritized problem
because this condition will only occur
if there is the occurrence of problems
which are not addressed first.
3 This is a potential problem because it
is not presently existing however it
may happen if there is lack of
information about how the problem
occurred and progressed. It is the
third prioritized problem because if
important informations about how
the condition occur and progress,
other problems might happen.
According to Maslows Hierarchy of
needs, Cognitive needs is on 5th level
on the pyramid and this include the
knowledge and understanding of the
patient about their health conditions.
4 Risk for fluid volume deficit is a
potential problem since it may only
occur if there is the presence of said
 conditions: diarrhoea, imbalanced
nutrition: less than body
requirements and knowledge deficit:
lack of information about proper
food preparation and handling. It is
least prioritized because it is not
presently existing and it will not
occur if the first problems are
addressed.
