SPC does not agree that this is a violation of PMC requirements, in that we did not follow our procedure

for test pile constructions.

The NCR (number) was generated because of the cavity formation and collapse on top layer of test pile
TP-02

Following we do having the same procedure practiced in other test pile Tp04,TP03,TP01

We have done a complete review of all the deficiency reports and procedures based on the sequence
below

Boring (0-26 m) 09-08-17 9.28 to 17.45

Soil level raised 13 m in borehole noticed 10-08-17 @ 7.45 am

Boring & cleaning 13 to 26 m 10-08-17 11.30 to 18.00 (boring activity interrupted due to
heavy rain for 6 hours

17.30 to 18

Soil level again raised upto dpth of 17m 11.08.17 @ 8.30

Cleaning 11.08.17 8.30 to 9.15

Boring 26-28.4 m 11.08.17 9.15 to 9.40

Cavity formation identified 11.08.17 @ 9.42 am

Decided to abandon 11.08.17 @10.15 am

The failure occurred due to unforeseen climatic and soil


conditions(surcharge from nearby steel area)

Heavy equipments operations nearby pile area and created vibrations is also be a reason of collapse.
Although all of the above works monitored by PMC it is very clear, we followed the best practices and
the construction time is becoming more than 24 hours because of unforeseen natural climatic
conditions

As noted in NCR report SPC will perform a review of all conditions,for the construction of new piles.

After further consideration ,SPC management has decided to issue a alternative methodology as a
corrective Action, to do a more review of Pile construction issues.

Further Actions and completion dates will depend on the results of new methodology and CAR
investigations.
