KATIE ADULT DAY CENTER POLICIES AND PROCEDURES

SUBJECT: Personnel Requirements and Records

POLICY: Personnel are in place and a personnel record on each staff person is
maintained in compliance with requirements of licensure.
PROCEDURE:
1. Center Director. A Center Director is in place who meets both the requirements in A
and B or only the requirement in C:
A. licensure by the state of Minnesota as a licensed practical nurse or completion of at
least two years of postsecondary education from an accredited college, university,
technical college, or correspondence school; and
B. completion of two years of paid or volunteer experience in planning or delivering
health or social services including experience in supervision and administration; or
C. completion of four years of paid or volunteer experience in planning or delivering
health or social services including two years of experience in supervision and
administration.

2. Personnel Standards: The standards and requirements in A-C apply to all employees:
A. Persons who supervise employees must be at least 18 years of age.
B. Employees or consultants who perform services that require licensure, certification,
or registration by the state of Minnesota must have current licensure, certification, or
registration in their field.
C. An employee, other than a physician, registered pharmacist, registered nurse, or
licensed practical nurse, who is responsible for medication assistance shall provide a
certificate verifying successful completion of a trained medication aid program for
unlicensed personnel approved by the Minnesota Department of Health or shall be
trained by a registered nurse to provide medication assistance in accordance with part
9555.9710, subpart 3.

3. A personnel file for each employee is established. Files are kept securely, in the Center
Directors office. Included in the personnel file are:
a. The employees job description.
b. An employment application or resume indicating that the employee meets the
personnel standards above.
c. Background study results

d. Documentation that the employee has completed orientation to the center as
required in 9555.9690.
e. Documentation of an annual performance evaluation.
f. Documentation of completion of the annual in-service training required in
9555.9690.
g. Documentation, when applicable, that the employee has completed the
cardiopulmonary resuscitation and airway obstruction treatment training required in
part 9555.9690, subpart 2, item C.
4. A personnel file for each consultant whose services the center purchases either by
contracting directly with the individual or by contracting for the persons services with
another organization. The file shall include:
a. A copy of a signed contract or letter of appointment specifying conditions and terms
or employments; and
b. Documentation that the person under contract meets any licensure, registration, or
certification requirements required to perform the services specified in the contract.
5. Personnel records are kept for 5 years after conclusion of employment.
