NOTE: Indicate by checking the appropriate rating () what you perceive as the rating that closely
approximates the seminarians external behavior. Before doing the evaluation, begin with a short prayer
involving the guidance and wisdom of the Holy Spirit. We do the evaluation NOT to put down any seminarian
but to help him grow in his vocation.

1. He shows diligence and interest in his studies by giving time for
reading, research, and well prepared reports.
2. He manifests good study habits and proper use of study periods.
3. He shows a basic capacity for comprehension, analysis, and
articulation of thoughts and insights.
4. He displays basic skills in oral and written communication in both
Filipino and English language.
5. He participates actively in activities that enhance his intellectual
formation, e.g., quiz bees, extemporaneous and writing contests,
debates, seminars, symposia, etc.
6. He is honest and prepares for examinations. He does not cheat in
quizzes, assignments, and examinations. He does not cheat in his
written reports, position papers, reaction or reflection papers.
7. He attends classes punctually and actively, participates in class,
e.g., passes assignments, reports, etc., on time.
8. He takes and passes examinations with average, if not, high
grades.
9. He respects the opinions, views, and suggestions of his fellow
seminarians.
10. He respects the professors and academic personnel.

Academic General Average Equivalent:

100% - 95% = 1; 94% - 88% = 2; 87% - 80% = 3; 79% - 75% = 4; below 75% = 5

NOTE: Irrespective of the academic general average, a formatee, at the end of the school year, or end of
semester, will receive under Academics a rating of 5 if he fails in any academic subject and 4 if he obtains
a line of seven grade in any academic subject.
