Guidelines for filling PF withdrawal FORMS (Form No.

19 & 10C)

FORM 19 (Application form for withdrawing Employees Provident fund)

1. Mention your Name as per pay slip (In Block letters).

2. Mention your Fathers Name or Husbands Name in case of married women.
3. Mention this address: - Accenture Services Pvt. Ltd., Plant 3, Godrej & Boyce Complex, LBS Marg, Vikhroli
(West) Mumbai 400 079.
4. PF account no. MH/BAN/45665/_______ (Please refer your Pay Slip).
5. Mention the date of Leaving Accenture
6. Reason for leaving service - You can mention the reason -> Resigned.
7. Mention your Permanent Address
8. Mode of Remittance should only be through your Bank A/c.
Mention your active Savings Bank A/C details which should be in your name, you can also mention your
Salary A/C but keep transacting with your salary account so that it can be maintained.

You can ignore Contribution for the current financial year Form No. 5 and Form no. 10

Back to the same page should have 3 signatures of yours (as per cross marks X)

FORM 10 C (Application form for Employees Pension Scheme)

1. a) Name of the Member (as per payslip) (In Block letters).

b) Name of the Claimant (This is applicable only in death case)
2. Mention your Date of Birth
3. Mention your Fathers Name / (or Husbands name in case of married woman).
4. Mention this address: - Accenture Services Pvt. Ltd., Plant 3, Godrej & Boyce Complex, LBS Marg, Vikhroli
(West) Mumbai 400 079.
5. Region MH, SRO Code: BAN
Estt. Code No. 45665
PF account number (will be on your Pay Slip after Estt code - 45665).
6. Reason for leaving service - You can mention the reason -> Resigned.
7. Mention your Permanent Address
8. Select option NO
9. Mention your family details.
10. Ignore this point (N/A)
11. Mode of Remittance should only be through your Bank A/c.
Mention your active Savings Bank A/C details which should be in your name, you can also mention your
Salary A/C but keep transacting with your salary account so that it can be maintained.
12. Write No

Below point no. 12 employee should sign the form (as per cross marks X)
Second signature on advance stamp receipt (as per cross marks X)

Employee can ignore the remaining part of the form.

KEY POINTS TO BE CONSIDER FOR PF WITHDRAWAL:-

Need to submit Form 19, Form 10 C & Cancelled Cheque leaf (only 1).

Cancelled Chq leaf should be of same bank of which you have mentioned the bank details in PF
Withdrawal Form otherwise PF Authority will not accept the PF Withdrawal Form.

Please mentioned your EMP code and contact No on the back side of the Cancelled chq leaf.

As per Employees' Provident Fund Scheme 1952 - Para 69, for withdrawing Provident fund there is
minimum 2 months (60 days) waiting period from date of leaving Accenture The PF Withdrawal
application can be submitted only after 60 days from the date of Resignation.

You will receive Acknowledgement copy for submission of PF Withdrawal Application, once we submit the
same to RPFC office.

Employee can check PF Withdrawal Status online on RPFC official website after submission of Form
www.epfbandra.org
