Examplar
The examplar that I have selected is the comprehensive health assessment as it includes the whole
body and being of the patient. Comprehensive health assessment is not just physical examination of the
patient but focuses on the spiritual and social aspect of the patients living. In order to understand the
patient, the health care team must understand how the patient views healthy living and what spirituality
mean to them The understanding that comes from making full person assessment will enable the
healthcare team provide the best care possible for the patient. When best outcomes are desired the patient,
the family, the cutlure and the spiritual aspects have to be recognized and assessed as well. The reason I
selected this project is because it reflects the nursing process from the beginning as it implements the
patient centered care in order to achieve better outcome and promote healthier living.

The exampler that I have chosen relates with the Essential IX because it reflects holistic patient
approach of care as discussed on #3 of the essential implement holistic, patient centered care that reflects
an understanding of human growth and development (Essential IX). The patient will be able to be

views that one owns. The nursing staff and all the healthcare team can provide patient centered care that
will satisfy the patient and the family. In order to provide the best care ones whole body and spirit and
cultural views have to be protected. To gain better communication and understanding allowing the patient
to express themselves in the way that they know or feel comfortable is always the way to develop better
patient and provider relationship. As it is portrayed on #5 care has to be filled with compassion and
backed with evidence based practice and research. The comprehensive assessment that was done shows
how important it is to understand the patient's physical problems but also understand the cultural and
When developing nursing care plan it is important to understand the level of knowledge that the
patient and family have in order to carry out the teaching that is being provided to them. #7 discusses to
provide appropriate patient teaching that reflects developmental stage, age, culture, spirituality, patient
preferences an health literacy considerations to foster patient engagement in their care (Essential IX). As
the health assessment that was completed in this course asking the patients age along with education level,
spiritual and cultural preferences even to gender and requirements of care to allow the providing team to
be able to provide education and explanation of the process of diagnosis. As it was covered through out
the comprehensive assessment whether family or self teaching is required. Understanding ones spiritual
views and learning about their culture will enhance the care that the patient receives and allows the
providers knowledge regarding differences. AS #10 on the Essential IX supports the facilitation of the
patient centered transitions of care whether discharge planning or admission process the family can be the
care givers that need to understand the requirements of the patient's needs and recommendations in order
to promote safe care for the patient will depend on the comprehensive assessment that is done on the
patient.
