MRI/TRUS FUSION TRANS-RECTAL AND TRANS-PERINEAL
PROSTATE BIOPSY: HANDS-ON COURSE.

INTRODUCTION:
Trans-rectal ultrasound guided (TRUS) sextant prostate biopsy has been the gold standard for
diagnosing prostate cancer (PCa). In recent years, multi-parametric magnetic resonance imaging
(mpMRI)-TRUS image-fusion targeted biopsy has shown greater accuracy on diagnosis of clinically-
significant PCa, either primarily or as repeat biopsy after a prior negative TRUS biopsy. Although
the mpMRI has rapidly gain popularity majority of the urologists are hesitant on MRI interpretation
and, therefore, performing MRI/TRUS fusion biopsy. As such, MRI/TRUS fusion guided biopsies and
treatments are often carried out by expert urologists or uroradiologists.
COURSE OBJECTIVES:
Learning Objective 1:
To understand the meaning of the multiple parameters of the MRI-prostate.
Hands on: To interpret mp-MRI and identify areas suspicious for prostate cancer. Full mp-MRI scans
are selected by difficulty level stratification.

Hands on: to optimize TRUS imaging using different US machines: Koelis, Philips, B&K.
Hands on: To perform prostate registration on MRI using different software: Koelis, Uronav, Artemis
Hands on: To perform prostate registration on TRUS using different software: Koelis, Uronav, Artemis
Hands on: To perform MRI/TRUS fusion biopsy using different software: Koelis, Uronav, Artemis
Hands on: To perform MRI/TRUS fusion TRANSPERINEAL biopsy using different software: Koelis,
Uronav, Artemis
mp-MRI: how to read mpMRI, practical tips, significance of each mpMRI parameter [T2-weighted
(T2w), diffusion weighted (DWI), diffusion contrast enhancing (DCE); MRI reporting systems: Prostate
Imaging & Reporting Data System Version 2 (PIRADS-v2) & Likert scale; MRI protocol (endo-rectal vs
pelvic; 1.5 vs 3 Tesla); MRI limitations & fallacies.
TRUS: TRUS is a core component of fusion biopsy. As such, we will discuss: TRUS technique,
interpretation of multi-parametric TRUS (gray-scale, color Doppler, contrast-enhanced) its role in
MRI/TRUS fusion biopsy.
MRI-TRUS image-fusion: process of image registration (rigid vs elastic), various available platforms
for MRI-TRUS fusion biopsy.
Prostate biopsy technique details: TRUS machine set-up, patient preparation, needle sterilization,
antibiotic prophylaxis/augmentation, minimize/manage complications (infection, bleeding, retention).
MRI/TRUS elastic fusion Trans-perineal vs trans-rectal biopsy: Pros & cons, techniques, outcomes
(cancer detection, complications). How to perform transperineal biopsy: local anesthesia, probe
positioning, needle tracking and other tips and tricks.
Adequate time for Q&A will be incorporated within the case presentations.
