Transition Plan

Student Summary

Sample student is a 16-year-old boy in 10th grade at Bryan High School with a

learning disability in basic reading skill, reading comprehension and written expression.

Sample student is one of the students that enjoys learning, especially loves the History

subject. Sample student is weak in English and struggling in Science particularly in

Chemistry and Physics areas, but overall Sample student is a bright student.

Related to education, Sample student has a dream career, which is to become a

physical therapist. He got this motivation from his father, because he is a physical

therapist. Realistically, Physical Therapy (PT) requires a lot more work than what it may

seem. Sample student is aware of the job type and responsibility that is associated with

PT. Sample student knows that he will not be able to get directly into a university after

high school; therefore, he would go to a junior college and from there take a transfer to

four-year university. Sample student may be interested in PT, but he is not aware of how

much schooling it may require. Sample student would need to visit a university and view

the degree plan to know which courses one needs to take in order to become a PT. By

establishing career goals and ambition, Sample student has been able to determine the

next steps in pursuing a higher education, which allows him to plan for success in future.

Like many young boys, Sample student also loves to spend time with friends and

family, participate in sports, and play video games. His favorite sport is football and he is

good at it. Sample student works out every day after school to keep himself fit for his

future career. Although Sample student has a concrete plan for his future career, he also

has a backup plan. As a backup plan, Sample student has partnership with his older

brothers business. Having a business is not common for some young teens, because
generally teens tend to depend mostly on their parents and just believe in enjoying their

time. Sample student may be effecting his education in a way while focusing on a

business, when he should be focusing towards his dream career. Sample student needs to

be made aware that in order to become a PT, one must be able to maintain at least Bs in

classrooms and have 3.0 GPA or above.

Moreover, families are important to most individuals, but only few individuals

prioritize the importance of family. Sample student is one of the students that believes in

his familys tradition, culture, values etc. Since Sample students father is a PT, Sample

student brought up the interest of becoming a PT and wishes to carry it out as a tradition.

Sample students intentions are great, but sometimes following someone elses footstep

may not turn out as an outcome he would wish to have. His dreams are to help others

maintain their physical health but being PT includes other aspects like writing and

reading as well.

Lastly, after discussing his future dreams, goals and career with Sample student,

his teacher and his parents, it was clear that everyone wanted Sample student to become

successful in his future life. Sample student is willing to work hard in order to achieve his

goals. He is aware of the support system that is provided to him for any assistance he may

require. Sample students special education teacher has made all the accommodations that

would fulfill his needs. His general education teachers have made modifications in their

instruction that would help Sample student and other students in classroom to be

successful. Sample students special education teacher believes that one should not only

focus on education, but they must take out time for personal enjoyment.







Supportive Tool For Informal Assessment

As a 10th grade student, Sample student is self-aware about his future career and

his education. An informal assessment Moving on to Your Future (Synatschk,K.,

Clark, G., Patton, J., & Copeland, R) was applied to help Sample student become more

aware about different types of careers and his interest. Through the attained information,

the student recognized his abilities and decided to further research on the medical field

and architecture landscape.

The student completed the Moving on to Your Future assessment. This

assessment is about knowing which career may best fit ones interest. On the Assessment

there are 8 different sections (clusters) with topics unknown; Sample student needed to

put checkmarks under each clusters statements that applied to him. At the end of the

assessment, directions stated to count the total check marks besides each cluster. After

completing this assessment the results were eye opening to Sample student. Sample

student completed the assessment and wrote the total counts of check marks besides each

cluster, he saw that there were many check marks that were equally distributed. This gave

Sample student the understanding that he may research the major career fields that he got

the most checks on.

In conclusion, through this assessment, Sample student was able to make a

decision on what exactly he wants to focus on for his future career. Before conducting

these assessments, Sample student wanted to pursue a career in physical therapy, but after

completing these assessments the student is determined to do little more research on the
medical career fields and architecture landscape. To decide on a future career, it is

important for one person to know what his or her interests are in life. Through these

assessments, Sample student has learned about different types of career and his interests

he may want to consider for his future career.


Informal Transition Assessment Outcome

Description

The informal assessment that was applied is the My Next Move by O*NET

Interest Profiler. The applied informal assessments helped Sample student become more

aware about different types of careers and his interest to choose a career. Through the

attained information, the student recognized his abilities and decided to further research

on the medical field and architecture landscape.

The student completed the online O*NET Interest Profiler at My Next Move.

This assessment possessed questions that ask individuals their likes and dislikes in

particular area that would lead them to certain types of jobs they may want to look into or

may have interest in.

Rationale:

Sample student did not require support in education, independent living and

recreational/leisure. The main focus that Sample student requires is support in choosing a

career. Online O*NET interest profiler at My next move helped the student become

more aware of different types of careers.

Result:

The assessment sorted out the career in best fit, great fit or none. Sample student received

best fit in the Enterprises section, which is about maintaining a business. Sample student

may in the future end up with his own career related to business. However, Sample
student was determined to stick with Physical Therapist (PT) and Architecture landscape.

Both of the careers were in the great fit category.

Conclusion:

Through the use of this assessment Sample student was able to make a decision

on what exactly he wants to focus on for his future career. The first time conversation

occurred with Sample student about his career, he wanted to pursue physical therapy, but

after completing this assessment the student is determined to do little more research on

medical career fields and architecture landscape. To decide on a future career, it is

important for one person to know what his or her interests are in life. Through this

assessment, Sample student has learned how his interest may impact certain career fields.

































Self-Advocacy Statements

Post-secondary education:

In classroom, Sample student will need to sit in the front row and ask the teacher

for detailed notes to be successful. Sample student will go to the teacher and inform

him/her that he has learning disability. Sample student would need to inform his teacher

that he learns best from visuals and hands-on activities.

Employment:

Sample student needs to inform his co-workers/supervisor to give him extra time

to complete tasks. Sample student does not like to be bossed by people that are at the

same level as him, he would need to inform his co-workers not tell him what to do.

Independent Living:

Sample student will have minimal financial support from his family if he lives

independently. Sample student will find a job and work as a part-time to pay for his

expenses. Sample student is great with savings and will be able to communicate with

landlord if he has any questions.

Recreation and Leisure:

Sample student has a calm nature; he does not prefer mentioning about his

disability to anyone. If Sample student has a problem with a person he will explain to

them he does not like that and if reason is needed Sample student will answer the person.

Sample student has a tendency to blank out when not understanding the conversation
therefore, he will tell his friends to rephrase a question or comment that he did not

comprehend.

Transition Outcome/ IEP Objective

Education:

Strengths in this area: math, science and history (average)

Weaknesses in this area: reading and writing (moderate/average)

How the students disability affects this area: the student is able to be

successful, but doesnt give in much effort. The causes may occur because of his

learning disability and ADHD.

Parent/guardian input: both parents are supportive

Student input: the student wants to graduate and continue with further studies in

medical field.

1. Outcome: Upon high school graduation, Sample student will complete

SAT testing with passing scores required for Blinn College.

Timeline:

Sample student will take the practice test at school. Spring 2018

How the students disability affects this area: being exhausted with writing and

reading information because of his learning disability and ADHD

Student input: he takes full responsibility for having a job and maintaining his

grades in school.

university.

valid drivers license after completing a state certified driving course.

Strengths in this area: enjoys working out and plays football often.

Weaknesses in this area: exhausting himself with all the sport activities

to his ADHD

Job Analysis & Narrative

Sample student has an interest in the medical field, particularly in Physical

Therapy (PT). A face-to-face interview was completed with Sample students father.

Sample students father was interviewed because he has been working as a PT for years

and he knows his son well. His father did mention that this field has lots of writing work,

reading and studying to do on daily basis. However, Sample student is not fond of writing

and reading. Sample student was not aware that writing work is occurring often more

than helping patients with physical strengths. The only aspect that was interesting to

Sample student was helping individuals with physical strengths.

Furthering the conversation, Sample students father mentioned about how many

years of studying he had completed, schools that he went to, types of work and time

commitment to Physical Therapy. Sample students father is well aware of Sample

students abilities and he knows that Sample student needs support in science, reading

and writing, but no matter what the end result may be Sample students father will

support his son in any possible way. As a pre-service teacher, choosing to interview with

a well-known PT provides better insight, as the interviewee is aware about the students

disability closely.

After completing the Interview with Sample students father, Sample student was

informed about the discussion in the interview. The interview went well and the PT

answered all the questions that are required in this field. Some examples of Topics that
were covered in the interview were Personal Requirement, Time/Travel Factors, and

Functional Academic Skill Requirements, etc. After informing the student about this job

career field, the student decided to look into another career field, which is landscaping.

The reason Sample student wants to now look into landscaping is because he is working

in that field part-time. Sample student will conduct a research on architecture landscape

to decide if that is what he wants to do.

Student Progress Summary

Sample student has been supportive and cooperative throughout the transition

plan. As a pre-service teacher, having the chance to learn about Sample students interest

and personality gave a great insight of what his wishes may be. After interviewing with

the parents, new information was provided that is, Sample students interest in History

subject and his fond in collecting rocks. The parents perception is that Sample student

might build interest in study of history related to nature. After learning about the interest

in study of history, as a pre-service teacher, information was passed to Sample student

about the career field of petrology, archaeology and anthropology. Given the information

of different career fields, Sample student was determined to stick with the medical field

or landscape architecture. Throughout the planning, Sample student at first wanted to

become a physical therapist, but now he has opened his mind to look into different fields.

Through the careers assessment, Sample student will do research on the medical field and

landscape architecture.

Sample student was doing well at the beginning of the year and suddenly he

started to drop his grades. As a future educator, there was a concern as to why there was

such a drastic change in Sample students grades. During the interview, questions related
to academics were asked to the parents to know if the parents were aware of the students

grades. The parents provided the information that currently Sample student is not doing

well and they know the reason behind it. The parents have observed that when it is the

time of football season Sample student is focused in school. Also, the parents mentioned

that to have Sample student do well in school he needs some sort of motivation. Example

of motivation is Sample student has a picture of his dream car in his room and his goal is

to have that car someday. Sample student looks at the picture every day and he is

motivated to save money for that car. Sample student is a bright student, but he needs

motivation for to become successful. Sample student will need to pass all his classes, if

he wants to continuously play football in school.

Job analysis helped Sample student have a clear mind about what the job will be

like. For the most part, Sample student has still maintained his interest in Physical

therapy, but at the same time Sample student wants to know about landscape architecture

and he will do personal research on both. After completing the job search, this search

build curiosity in Sample student about schools that will be great for him to join after he

graduates from high school. Sample student compared physical therapist and landscape

architecture fields and learned the differences between yearly pay, years of studying and

the tuition cost.

As far as other areas of transition, for independent living Sample student will be

living with his parents after he graduates from high school, if he decides to go at Texas

A&M University or Blinn College. If Sample student wishes to go out of College Station

for another school his parents will provide him with minimal financial support. Sample

student is aware of his Post-Secondary education goals, he will advocate for himself as
needs to meet with his requirement. Sample student is currently working as part time in

his business of landscaping and will continue until after high school graduation. Also,

Sample student enjoys working out; therefore, he will maintain his fitness after high

school graduation. At last, Sample student will determine the career field he wants to go

into after he completes a personal research on physical therapist and landscape

architecture. As a pre-service educator, a suggestion was provided to Sample student and

that was to look out for different fields that are in his interest.
