CONSENT AND RELEASE OF LIABILITY FORM

declare that:

1. I am a student of Silliman University College of Law for the school year 2017- 2018;

2. In line with the BAR Examinations to be held at the University of Sto, Tomas, I will
join the SU Law Bar Operations in Manila this ________________ until _____________
to assist to the needs of the BAR examinees;

3. My decision to join the said activity is purely voluntary and I affirm and acknowledge
the inherent hazards and risks associated with the same;

4. In view of the foregoing, I hereby free and release Silliman University and/or its
officers/employees from any and all liability, civil, criminal or otherwise as a
consequence of any injury or damage that I may incur or sustain in the course of the
activities;

5. I execute this waiver voluntarily knowing its full import and consequences.
