1. Chief Complaint

2. History of Present Illness

The patient was just sitting on her chair when suddenly her
spouse noticed that the patient was out of breath, it was shallow
and rapid. After monitoring the patients blood sugar (88 mg/dL)
and vital signs they noticed that she cannot open her eyes and
respond anymore letting them decide to rush the patient to the
emergency room.

3. Past History

The patient has type 2 diabetes mellitus and she is

hypertensive. Since the 1990s she experienced an excruciating
pain on her left knee. The pain wont go away so they decided to
have a knee replacement surgery at the United States of America
which left her a 20cm scar. At the 1990s also she noticed that a
mass is growing in her left breast, they undergone biopsy and it
was a benign tumor. She undergone mastectomy reasoning that they
dont want it to recur and to decrease the risk for breast
cancer, the surgery left her with a 19cm scar on her left breast.
