RE : NOTICE OF DISCIPLINARY ACTION

On 12 December 2011, the Company issued a PREVENTIVE SUSPENSION letter


against you, charging you for violating the Companys rules and regulations as
follows:

1. Failure to report Since 07 to 15 November 2011 as Sectorized Personnel


whose assigned at Sector 11 PAHRI Gate Assist as NIGHT SHIFT GUARD
without informing your detachment officers on your whereabouts;

2. Failure to report Since 16 to 30 November 2011 as Gater Security Personnel


whose assigned at Main Gate (DAYSHIFT) for the whole shift without
informing your detachment officers on your whereabouts; and

3. Unlawfully using the identities of fellow employees, without their consent


and knowledge, in order to contract loans for your own benefit.

As a result, you were issued a preventive suspension for thirty (30) days pending the
Companys formal investigation of the abovementioned charges. In turn, you
submitted a written explanation dated 13 December 2011 detailing your side.

Unfortunately, after considering the evidence, we are more inclined to believe the
testimony of Shift Security Officer _________________________ [MR. DIAZ] when he
affirmed that indeed you failed to report on the abovementioned dates without
informing your detachment officers in violation of Company policy.

Furthermore, testimonies of your fellow employees namely,


_________________________ state that you had unlawfully, without their consent or
knowledge, used their identities to contract loans for your own benefit.

On this score, the Company is constrained to TERMINATE your services for your
abovementioned actions. You are hereby ordered to vacate the premises after
receipt of this correspondence.
