1. GENERALITIES
1.1 Scope
The Plan present contains elements and standards of strict fulfilment for the employees and workers of our
company. HOWEVER AGREE TO COMPLY WITH THE PROGRAMME, PROCEDURES AND PRACTICES
OF SAFETY, HEALTH AND ENVIRONMENT OF OUR CUSTOMER DURING THE DEVELOPMENT OF
THIS PROJECT.
1.2 Objectives
The objectives of the present plan are:
Inform and norm the aspects Environmental, Health and Safety in the work for its respective execution.
Include an internal technical/normative instrument to carry out every task according to the practices and
disciplines of Safety and employment Health established by the laws, regulations and standards of our
client.
2. LEGAL REQUIREMENTS
This Plan is based on the following documents:
Occupational Safety & Health Administration (OSHA) 29 CFR 1926 of the United Status of NorthAmerica.
Mine Safety and Health Administration (MSHA)
National Fire Protection Association (NFPA)
Enviromental Protection Agency (EPA)
HSE Project Health, Safety and Environmental Rules &Regulations of SAIPEM.

3.1 Mission

4. POLICIES OF WORK
4.1 Policy of Meetings and Training

Meetings of five minutes: before the works begin, the Supervisor will give a chat from 5 minutes to their
personnel, orienting them on the sure work that will perform. Any worker that have some doubt should ask
his Supervisor and this should resolve the problem. Every chat and meeting of safety that give should be
communicated in writing in the corresponding formats, causing signing to each one of the workers as sign
of conformity.
Meetings weekly: Every week a meeting of Safety will be performed, Hygiene and control of the
Environment that will have a most minimum duration of thirty minutes, in which they will intervene the
Project Chief, Project EH&S Manager, Supervisors and workers in general. This meeting should be
documented in a format of weekly meeting.
Training HAUG S.A. will qualify and will coach its personnel in the different related areas from EH&S.
Instruction to third parties every visit or supplier of HAUG S.A. that have that to enter to Project, previously
will be instructed on the existing risks and the administrative norms and of EH&S that should comply.
4.2 Policy of Use of Personal Protective Equipment (PPE)
Safety glasses, hardhats, hi viz vest and steel toed work boots of safety will used by the personnel during the
workday compulsory, and according to the degree of risk of the work be considered, will be offered special
implements. The administrative workers that be found labouring inside the offices are excepted of this
disposition.
Safety of the head: The head is the most important part of the human body, that is why the use of the hardhat
for its protection is permanent and strict in project during the workdays. It prohibited in work having loose hair
only if it is long , it should be maintained collected in such a way that not cause danger since it could be
trapped by machines or equipment in operations.
Safety of the eyes: Safety glasses during the continuance in the work will used, except for works in offices.
For the special works of welding, brazing, cutting and other will be provided of the adequate implements.
Protection of the feet: For the safety of the feet is obligatory the use of steel toed work boots to avoid
accidents. Its prohibited the use of slippers, sandals or similar. The boots of rubber with steel toed are
permitted in humid places or contact with the water. The electricians will use dielectric boots of Safety.
Uniform: All personnel will use a hi viz vest during the permanence in the project. According to the requests of
the project or work the company will provide the personnel work uniforms. This should be comfortable and
tight of such form that cannot be stalled neither to be hooked in machines and equipments of construction,
structures, mobile parts, etc. Moreover, workers should not use shirts, pants or very wide overalls or deform.
Clothing of synthetic fibber will not be used.
Ear safety: All personnel will use protectors of ear in places where the noise surpass the 85 decibels dB, such
as: compressor, pneumatic hammers, blows of constant bulge, etc.; and in those places that apply hand
grinds , workshops where there are noises of machines and vehicle.

Safety of arms and hands: according to as correspond should be used protection for the hands and arms.
Previously an evaluation of risks will be made in order of determining the type of gloves to use.
Safety of the face will be used the adequate mask in works of chemical materials, welding, cutting, brazzing
or in those machines that throw shaving, dust or abrasive.
Respiratory protection: it will be provided to each worker respirators adequate for each work that imply risk
or excessive concentration of dust, smoke, gases, fog, vapors, toxic emanations or by deficiency of oxygen in
places confined. Before using the respirator, be assured that he has been trained about it.
Protection in works of height: Is obligatory the use of the safety harness in works of height, equal or more
than 1.80 mts. The infringement of this norm is motive of drastic sanction.
Inspection of the equipments: All the personal protective equipment will be inspected, codified and repaired
or placed out of service.
4.3 Policy of Order and Cleaning (Housekeeping)
The order and cleaning is a very necessary and fundamental activity of individual responsibility of each
person.
The places of work, walkways, stairs and all the other areas should be maintained free of filth.
All the wastes should be placed in cylinders placed in different places.
The ways, halls and other places where the personnel circulates should be maintained cleared of
obstructions and other materials that can cause any accident.
The stores and deposits should be maintained clean and orderly.
Every building material, cords, wires, electric cables they should be far away of the surface of circulation of
the personnel, these should be placed in high places over the surface in order not to cause damage to
personnel or equipment (or protected).
It is compulsory to keep each work area in order and clean.
The sanitary services and dining rooms after being cleaned should be disinfected daily to avoid infections.
4.4 Policy of Health and Other Services
Every worker that enter to project should be submitted to medical exam.
In every installation or work, the personnel should include adequate dining rooms and enclosures, which
they should be maintained always clean, free of wastes.
The personnel should maintain the good judgment and discipline in the dining rooms, sanitary services and
internal areas of Site project.
The portable chemical baths should be maintained clean, disinfected and in good conditions
4.5 Policy on Control of the Alcohol and Drugs

HAUG S.A. is conscious that the use of alcohol and drugs in the workplace affects the normal health of the
workers to carry out their tasks safely for themselves and others. That is why it is prohibited to consume or to
possess alcoholic beverages and drugs during the work day or while working in the site projects.
Measures to take:
Carry out tests of alcohol tests to the people found involved in a serious accident.
The Supervisor will control the behaviour of his workers in alcohol consumption signs search and drugs.
Or in his defect he will be utilized instruments like the ALCOTEST for this purpose.
In case it has been proved that the worker has consumed alcohol or drugs, the coordinations for
respective analyses will be done.
The worker that is consuming drugs by medical prescription, will report this fact to Human Resources
Administrator.
All the workers of site projects, without exception, will comply with their responsibilities that are indicated in
our politics on the use of alcohol and drugs.
Personal will be sanctioned, any be its hierarchy yes infringes these norms, in agreement al item 4.6 of the
present document and legal devices in force.
4.6 Policy of Incentives and Disciplinary Measures
Incentives
HAUG, will establish safety incentives programs for the workers consisting on the following points:
Monthly prize to the workers that there are outstanding in safety.
Celebration of the ACCIDENTS ZERO goal.
Disciplinary Rules
All the rules, norms and procedures, contents in the present plan are of strict fulfillment by part the workers,
without restriction of charge or category.
In agreement with Internal Regulation of Personnel of HAUG and legal devices in force, that break the
dispositions given will apply the following sanctions:
Verbal Admonition.
Admonition written
Suspension
Dismissal
If the misdemeanour is serious, the worker will be fired not including the sanctions before described.
Serious misdemeanour in Safety is considered :
Cause serious wound or death, which makes the worker be under the disposal of the competent
authorities.

5. RESPONSIBILITIES
5.1 Project Chief
Is the direct head of all the operations in Project and therefore of the Safety, Health and Environment in the
place. Consequently, he must provide the resources and facilities needed to avoid any occurrence of
incidents, damage or waste of the resources of the business.
Will set a personal example and assist in the proactive promotion of EHS as a personal objective
Implement, comply and to make fulfill the EH&S policy, plan, objectives and designated goals in the
project.
It reviewed all the methods and procedures of work, verifying the compatibility with in EH&S Plan.
To participate actively in the EH&S initiatives.
Conduct inspects all the areas of the Project.
To make weekly EH&S meetings with all the workers of the Project.
Participate in accidents investigations.
Review and improve permanently the EH&S programs of project, in agreement with the updates of the
laws, standards and technologies in the matter.
5.2 Project EH&S Chief
Administer the EH&S Plan of project
To advise to the different areas form the project in EH&S aspects
To offer the EH&S induction training to all the new workers in the project
To train all the personnel in EH&S.
Inspect weekly all the areas of the project, review the results to identify EH&S issues and deficiencies and
to advise supervisors of findings.
Lead Job Safety Analysis reviews.
To participate in accidents investigations.
To provide weekly reports of EH&S project a our client e internally to HAUG S.A.
To participate in the EH&S meetings organized by our client.
To offer first aid to rough personnel.

Be fully conversant with our client EH&S Rules for Construction, standards and the project EH&S
requirements.
Evaluate the EH&S performance of all the areas e inform a Project Chief.
5.3 Erection Supervisor
It planned and it implemented appropriate methods of agreed work with the safety standards.

Conduct EH&S inspections weekly in all areas of project with the goal to verify the fulfillment of the
standards and EH&S plan and to take the corrective measures in case that some norm is being infringed.

To participate in accidents investigations.
To participate in the audits of Safety to order of the Project Chief or of the Client.
Arrange continuously the improvement of the working conditions to prevent accidents and employment
illnesses.
Verify that the Area of Maintenance comply with the preventive program of maintenance, in order to
assuring the operability of equipments and tools.
5.4 Quality Supervisor
It planned and it implemented appropriate methods of agreed work with the safety standards, specifically in
the works that it corresponds to him.

5.5 Human Resources Administrator
Verify that every new worker enters to the business with the certification of pre medical exams
employment.
To offer each worker the services of social insurance ESSALUD and complementary insurance. In every
case these services should be maintained brought up to date.
To offer the facilities like feeding and others with he purpose of maintaining an atmosphere of healthy and
healthful work.
To transact the necessary documentation for the entrance of the workers to the project.
5.6 Planning
To make the planning of all the activities of the project contemplating the aspects of security, health and
environment during all the work.

Maintain the stock of personal protective equipment such as hardhats, protectors for ears, steel toed work
boots, gloves in their diverse uses, high-viz outer garment, facial protectors and safety glasses. This stock
should be compatible with the quantity of the scheduled personnel, in such a way that all they include these
requests.
Order that the warehouse includes enough fire extinguishers and to inspect them.
Maintain order and cleaning in the warehouse.
Order that every material or flammable product, chemical or hazardous materials be stored in other
different areas al warehouse of materials. To maintain a file with MSDS (Material Safety Data Sheet) of
existing dangerous products in the project
Every acquisition of equipment, implement or material of safety should include the review and approval of
the Project EH&S Manager. Similarly, it will be done with the dangerous materials and products.
Order that the warehouse carry a KARDEX everyday on the safety equipments delivery to the workers.
To inspect equipments, tools and other objects of warehouse, placing to the elements in good state the
tape of the color code corresponding to the trimester in course (yellow).

Comply the programs of preventive maintenance of machines, equipments, installations and electric tools.

5.10 Workers in General

Obey the dispositions, norms, rules and instructions that give the company and Supervisors.

Employees are responsible for their personal safety and the safety of their co/workers, through both their

All worker of HAUG S.A. will obligatorily receive a Course of Induction to the New Personnel, before entering

Also all worker will obligatorily receive the Course of Induction offered by our client, before the beginning of
the workings in the project.
Daily Meetings of 5 minutes
They are offered to all the personnel of the project, being distributed by the Supervisor of area. In these char
them will touch to agreed subjects to the hazards and existing risks in the work area.
Weekly Meetings
It will be realized weekly during 30 minutes by the Project Chief altogether with the Project EH&S Manager.

Training Procedure
In function to the occupational hazards and risks that they appear in the development of the workings will
choose the subject to char it it of 5 minutes, orienting to them on the safe work that they will carry out.
It is necessary that the exhibitor previously reviews information related to the subject of selected it char,
being able to mention norms, histories and anecdotes, among others.
Later it will prepare the content of its exhibition and the form of teaching, in addition to some resources to
use (if outside the case) like audio-visual means, among others.
The exhibition of the subject will be made in an suitable atmosphere, initiating with a brief introduction and
specifying what it is going away to treat.
Later the subject will appear counting on the active participation of the workers by means of its

12. SAFETY STANDARDS
HAUG S.A. comply strictly standards of our client during a activities in the present project.
12.1 Housekeeping
The order and the cleaning constitute fundamental activities that they must be executed by all the workers
who toil in the project.
Good housekeeping is an important element of accident prevention.
Good housekeeping will be planned at the beginning of the job and be of a primary concern of all supervisors
and workers throughout the entire project. This will be a part of the daily routine and clean up will be a
continuous process.
Some basic rules to consider are:
The areas of work, passages, stairs and all the other areas will stay free of obstacles, equipment, and
materials.
In diverse sites appropriate containers for sweepings will be placed, which will be used for the positioning
of remainders.
The liquids (such as paintings, reliable, dissolvent, oils, and fats) and any other material or containers that
have contained chemical, will be rejected in agreement with the procedures and regulations established
for toxic wastes.
The storage areas will stay cleanings and the materials properly stored.
The electrical cords, wires, cables, hoses, and other temporary systems, will stay outside the surfaces to
walk and in a high position.

All the pieces of wood, rejected material and sweepings will have to be removed immediately of the work
area.
All the remainders of reliable, oily rags, and incendiary fuels, will stay in containers closed resistant to the
fire until they are retired of the work site.
12.2 Personal Protective Equipment
Previous at the beginning of the workings, it will be made an identification of hazards and evaluation of risks,
with the purpose of determining the personal protective equipment to use.

All workers will be trained in the storage, use, cleaning and spare part of personal protective equipment.

It is obligatory the use of safety glasses, hardhats, hi viz vest, steel toed work boots of safety and uniform of
work at any moment in the work fronts and where risk against the security and health of the people exists. In
case of appearing additional risks the suitable implements will be used to control these.

All personal protective equipment to use in the project will fulfil the stipulations of the pertinent norms and will
count on the certification of respective quality.

Double eye protection is required for certain operations such as welding, cutting and brazing.

All the personnel will use auditory protective equipment in the designated areas that exceed 85 dB.

Fall protection is required for employees working above/below ground when not protected by an approved
scaffold or staging.

All respiratory protective equipment will count on the approval of the National Institute Occupational Safety
and Health of EE.UU.
The respiratory equipment will be used, stored and maintained in accordance with the requirements of the
manufacturer.
The respiratory equipment will be selected with base in the risks to which the worker will be exposed.

The use of steel toed work boots of safety is required during all the works in the project.

The equipment that has undergone any type of alteration and/or that is in inoperativo state will be retired of
use in immediate form.

12.3 Tools in general


Every tool should be used only for the purpose that was designed.
The tools should be maintained in optimum conditions of operation and use and they can just be used by
qualified personnel.
Mechanic portable Tools
The portable tools should be operated for qualified and authorized personnel.
These tools should have guards and adequate protections.
Read the instructions of warning installed in each machine of the manual of operation and to comply with
their content.
It is indispensable that the Supervisor give the instructions before the use of the tool.

Electric Tools
The welders of electric arch, turbines, drills, are electric tools that HAUG S.A. will utilize in the project.
Utilize both hands, to have good base, to utilize the tool according to the instructions that the Supervisor has
given.
Any failure of the tool should be communicated to the Supervisor or electrician.
Use always facial protector or lenses of Safety when you are operating machines or electric equipments.
All the machines and electric tools should be operated just for what they were designed; do not withdraw the
protector, guard or another element of Safety.
Do not used tools that have cables wore out or in bad conditions
The tools and electric machines should be connected to land.
The tools damaged or faulty should be retreats of the use and will be put them a placard that says DO NOT
USE
Remains prohibited repairing electric tools, this work concerns to the technical electrician

Hand Tools
The faulty tools should be returned to warehouse
They should be marked or to be identified of way such that the user or owner can recognize it.
Never use a hand tool to do lever. Neither forces its capacity.

12.4 Electrical Aspects

All electrical work, installation and wire capacities shall be in accordance with the pertinent provisions of the
National Electrical Code, ANSI, OSHA, or equivalent in-country standards.
All electrical wiring will be carried out by qualified personnel.
All temporary electrical wiring will comply with the National Electric Code.
All portable electrical equipment will be grounded.
All single-phase low-voltage extension cords will be three-wire conductor type.
All low-voltage hand held portable electrical equipment will be used in conjunction with GFCI's (Ground Fault
Circuit Interrupters).

All temporary power panels shall have covers installed at all times. All open or exposed breaker spaces shall
be adequately covered.

Contractor shall develop a ground fault program in accordance with requirements established by applicable
regulation. In the absence of ground fault protection, Contractor shall establish a monthly-assured grounding
inspection program to include color-coding. The color codes used on this project are:
From January to March - Yellow
From April to June - Green
From July to September - Red
From October to December - Blue
(The cycle of colors is repeated for the next year).

12.5 Scaffolds
Will be established norms that guarantee that the scaffolds and work platforms systems be installed, they
inspect, use and be disarmed according to safe practices and the pertinent rules. Nobody, will modify, alter
or damage any scaffold. The modifications which are required will be performed only under authorization.
Scaffolds will only be erected, modified or dismantled by qualified scaffolders.
All scaffolds comply with standards of our client.
Every scaffold must be reinspected and certified for use every 7 days and will use a scafftag system.
The use of harness is obligatory when it works in scaffolds, which will have to be hooked to a safe point
outside the scaffold. Also the use of elastic chin straps of plastic is obligatory.

Scaffold erection will be in compliance with OSHA 29 CFR 1926.450 or equivalent in-country standards.
Workers erecting, dismantling or using scaffolds shall be trained in accordance with applicable regulations.

Scaffolding shall be erected, moved or dismantled under the direction of a Competent Person qualified in
scaffold erection.

Footings or anchorage for any scaffold shall be sound, rigid and capable of carrying the maximum intended
load without settling or displacement.

No unstable objects such as concrete blocks shall be used to support scaffolds or planks.

Any part of a scaffold weakened or damaged shall be repaired or replaced immediately.

All scaffold planking shall be free of knots and cracks (Class A number) and shall completely cover the work
platform.

Scaffold planks shall be laid tight cleated at both ends or overlapped a minimum of 12 inches (300 mm) and
nailed or bolted to prevent movement, with overlaps occurring directly above scaffold supports.

Safe access shall be provided to the scaffold platform. Specifically, a ladder with a safe means of access to
the platform from the ladder.

Scaffolds shall be equipped with a top rail made of lumber not less than 2 x 4 inches (50 mm x 100 mm) (or
equivalent strength). Minimum top rail height 42 inches (1 m) with a mid-rail of 21 inches (0.5 m) high made
of lumber not less than 1 x 6 inches (25mm x 150 mm) (or equivalent strength); and toe-boards shall be
installed on all open sides and ends of scaffold platforms 10 or more feet (3 m) above the ground or floor for
scaffolds where the guard rail is the primary means of fall protection.

Scaffolding not adaptable to guard rails shall require the use of a safety harness with two lanyards attached
to a secure substantial object.

Mobile scaffold casters shall be secured and locked prior to mounting.

No personnel shall be on mobile scaffolding when it is being relocated.

Contractor employees working swing stages, boatswain chair, floats, suspended scaffolds and needle beam
scaffolds, etc., shall wear safety harnesses with two lanyards attached to an independent lifeline.

A proper scaffold inspection and tagging system shall be instituted and maintained by the Contractor erecting
or using the scaffold.
12.6 Ladders

The use of ladders with broken or missing rungs or steps, broken or split rails or other defective construction
is prohibited.

Ladders shall extend no less than 36 inches (1 m) above landing and be secured to prevent displacement.

Portable ladders must be equipped with safety shoes.

Wooden ladders shall not be painted.

Metal or conductive ladders shall not be used on the Work site.

Ladders shall be inspected regularly and records provided by the Contractor to Company upon request.
12.7 Material Handling and Storage
The manual manipulation refers to the task that the force of the worker is carried out by its means. It should
be taken into account the following norms:
Use adequate gloves to avoid cuts or sharp pains in hands.
If the load is heavy request help to raise it; but it is preferably recommended the use of mechanical carts.
To raise a load do not use the lumbar musculature. Employ the effort with the leg musculature of
maintaining the back straight.
Reinforcing steel is to be stored in separate piles according to size, length, and placement sequence.
No Smoking and Warning signs as required for the stored products will be posted and maintained.

12.8 Hazardous Materials


EH&S area will support responsible of warehouse about the degree of hazard of the material or substance,
consequently the adequate material for the different processes of the production will be acquired.
The responsible of warehouse will be performed weekly inventory of the hazardous materials of project.

The management and utilization of the hazardous materials will be according to the indications of the
manufacturer or supplier. It will serve to instruct the workers by means of the sheets of instruction of the
manufacturer or Material Safety Data Sheets (MSDS).
For the storage of these materials previously should be coordinated with the Project Chief to adopt the
measures of the case.
Every container that has contained hazardous materials should be signalized.
A Hazardous Communication Program will be implement and spread a personnel of project.
Material Safety Data Sheets (MSDS) in both English and Spanish will be maintained at the job site and will
be available for examination from the EHS Supervisor.

12.9 Fire Prevention and Protection


It is prohibited final to smoke in the zones of work. Warnings of prohibition will be placed to smoke and it will
instruct the personnel previously.
A procedure written of storage, use and disposition of inflammable materials will be elaborated.
The work area will stay clean, ordered and free of combustible materials and incendiary fuels.
Fully charged and operative fire extinguishers will be supplied according to the standards of our client.
Personnel are required to be familiar with fire extinguisher locations and operation.
Personnel will be trained in the correct use of each type of extinguisher on the site.
Extinguisher will be inspected monthly by the Supervision.
12.10 Welding and Cutting
All the electrical machines must operate as they are designed; it does not have to retire the protector, keeps
or another element from safety.
Machine weld that have spent cables or in bad conditions do not have to be used.
The electrical tools and machines must be connected to earth.
The damaged or defective machine must be retired of the use and a poster will be put to them that it
indicates NOT TO USE.
It is prohibited to repair electrical tools; this one work is incumbent on to him to the responsible for
maintenance.
The positioning of screens is necessary to protect to the rest of workers of the radiations caused by the
electrical arc.
Fully charged and operative fire extinguishers will be supplied in the work area.
Frames of electric welding machines operated from power circuits will be effectively grounded.
Electrode and ground cables will be supported to prevent interference with workers.
Defective cable shall be repaired or replaced.

An electrode holder shall be properly rated and insulated.


Electrodes shall be removed from the electrode holder when unattended.
The welders will use a necessary Personal Protective Equipment for the welding and cutting.

12.11 Unfired Pressure Vessels


The cylinders of oxygen or fuel should not be rolled thrown horizontally (they should not be used as rollers).
For the transportation utilize appropriate cars or to roll them vertical. They should neither be raised with
crane or truck crane, freight elevator. The raise of the cylinders should be manual or with appropriate cages.
The cylinders should be stored of the following form:
Hitched in vertical position with chain of average inch with the cap placed.
The environment of storage should be remote, aired, with ceiling and enclosure with netting wire of
preference.
Fire extinguishers should be placed in the quantity required.

horizontal plane at which a material will lie without sliding), or shall be adequately shored.

Ladders or steps shall be provided in all trenches 4 feet (1.2 m) or more in depth. Ladders or steps shall be
located to require no more than 25 feet (7.5 m) of lateral travel before having access or egress.

Material excavated shall be stored at least 2 feet (600 mm) from the edge of the excavation or trench and
shall be shored to prevent material from falling into the excavation.

Cylinders shall not be hoisted by magnets or choker slings. Valve protection caps shall not be used for
hoisting cylinders.

Cylinders shall not be placed where they can become part of an electrical circuit.

will propose improvements (if there were them) fulfilling the cycle of continuous improvement of the company

17. HEALTH CERTIFICATES


Subcontractor must provide for all its personnel utilized during the Services, a Health Certificate as proof of their
fitness for their respective tasks in relation to the Services.
The said Certificate must be presented Snamprogettis assigned representative and also given to its Medical
Officer on site prior to their actual involvement in the Services.
Unless otherwise stated, the validity of the certificate shall be for one Year.

Snamprogetti representatives will have the right to refuse Subcontractors personnel if not in possession of valid
certification.

6 OFFSHORE CERTIFICATION REQUIREMENTS


All Subcontractors personnel, which are involved in offshore activities, must have an offshore survival certificate,
which must be valid and in compliance with STCW and/or OPITO (Basic Offshore

Safety Induction and Emergency Training) and/or OLF (Basic Safety and Emergency Training Course).
Copy of the certificates of personnel scheduled to work offshore within the subcontracted activities shall be issued
to the Snamprogetti onshore organization prior to arrive at the embark location.
Original copy has to be carried with all the employees going on board Snamprogetti offshore units.
Snamprogetti representatives will have the right to refuse Subcontractors personnel if not in possession of valid
certification.

7 HSE DOCUMENT TO BE PROVIDED DURING THE BIDDING STAGE


The Subcontractor is required to provide details of his approach to HSE.
To this aims the following evidence have to be provided together within his offer.

a) submit a copy of your HSE Policy statement and advise how your companys commitment to your HSE Policy is
demonstrated;

b) itemize the methods by which your HSE Policy has been drawn to the attention of your employees and detail the
arrangements for advising employees of change in your HSE Policy;

c) submit a copy of your HSE Management System Manual;

d) provide a list of the last 5 (five) years safety statistics (world wide) within your organization. The list should
indicate the number of Fatalities, Worked Man-hours, Lost Time Injuries (LTI), Lost Time Injuries Frequency Rate
(LTIF), Work Restricted Cases, Medical Treatment Cases, First Aid Cases, Occupational Health Diseases and
Near-Misses. Where appropriate, separate statistics should be given for different locations. You should also
provide an explanation of how the types of accidents and near-miss incidents are classified and an explanation of
which categories of personnel are included within you safety statistics. Any other pro-active indicators the
Subcontractors want submit will be considered.

e) provide details of the pre-employment and periodical medical fitness examination;

f) in accordance with activities to be performed, submit your proposal of Project HSE Plan that is in compliance with
Snamprogetti HSE Plan and HSE local and international legislation. The Plan shall address at least the following,
but not limited to, items:
HSE Objectives and Targets;
Organization and Responsibilities;
Communication System and Methods;
HSE Induction and Training;
Hazard Identification and Risk Assessment;
Accident / Incident Investigation and Reporting;
Emergency / Evacuation Procedures;
HSE Audit and Inspection;
List of HSE Procedures applicable to the scope of work.

g) provide details of your both employees orientation program and HSE training program. How often are formalized
programmes conducted? Is safety training given also, for specialized operations?

h) provide a description of how significant environmental aspects will be identified prior to and during work
execution;
i) what arrangements does your company have for provision and upkeep of Personal Protective Equipment (PPE),
both standard issue and that required for specialized activities;

8 HSE DOCUMENT TO BE PROVIDED UPON CONTRACT AWARD

HAUG S.A. is required to provide, within 30 days upon contract award, the following information and supporting
documentation:

submit a specific Project HSE Plan and HSE Manual dedicated to the activities to be performed;
submit a specific Risk Assessment;
submit a HSE Procedures, comprising all the appropriate procedures for the subcontracted scope of work;
submit a Emergency Procedures, covering all the emergency situations that could arise from the specific of the
activities to be carried out;
submit a complete Personnel Training Program, dedicated to the scope of work.
