AFFIDAVIT OF PARENTAL CONSENT

I, JENNY ROSE B. DAVID, of legal age, Filipino, married, a resident of Purok 4

Brgy. San Isidro, Sto. Domingo, Albay, after having been duly sworn to in accordance
with law, do hereby depose and states that:

1. That I am the biological mother of MENARD GABRIEL DAVID and was born on
November 25, 2004. Attached is a copy of his birth certificate as Annex A;

2. That I am separated in fact with Menardo C. David, my husband and biological

father of Menard since 2013 and we dont have any communication to him;

3. That I freely and voluntarily give my consent to my mother, MARIETTA BATAS,

brother, BERCELITO BATAS, JR. and his wife CHRISTINE JANE BATAS, to
accompany my son Menard Gabriel David to Macau for a vacation on April to May
2017;

4. That during such vacation, they will shoulder all the expenses of my son needed for
vacation;

5. That this affidavit is executed to attest to the foregoing statements and for
whatever legal purposes this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 25th day of November,
