STEPS TO DO BEFORE ENROLMENT

STEP 1 – Render 8 hours of work per parent or

donation.

STEP 2 – Present Brigada slip to the enrolment in

charge.
STEP 3 – Wear proper hair cut for boys and no

colored hair for boys and girls.

STEP 4 – Present the following requirements:

For Grade 7
Form 138 (Report Card)
Photocopy of NSO or LCR Birth Certificate
Certificate of Good Moral Character

For Old Students

 Form 138 (Report Card)

For Transferees
Form 138 (Report Card)
Certificate of Good Moral Character
Photocopy of NSO or LCR Birth Certificate

For Senior High School

Form 138 (Report Card)
Certificate of Good Moral Character
Photocopy of NSO or LCR Birth Certificate
Photocopy of Certificate of Completion
STEP 4 – Fill up Enrolment Form

STEP 5 – Submit fully accomplished form

STEP 6 – Receive library slip.
