Supply and Delivery of School Furniture: 579 Units of Elementary Armchairs for Grade V-VI (all
wood), 90 Units of Secondary Armchairs (all wood) and 15 Teachers Table and Chair Set (all wood)

Gentlemen,

We respectfully write the Honorable Committee to seek the reconsideration of the letter dated February 28, 2012,
which states that our company has failed to pass the post–qualification process upon the ground that the notary
public who signed the Omnibus Sworn Statement as required in ITB Clause 12.1(b)(iii) is only commissioned until
December 31, 2011.

With all due respect to the Honorable Committee, we humbly entreat the reconsideration of the above finding upon
the following grounds:

1. The notary public who notarized the Omnibus Sworn Statement, Atty. Salvador T. Sabio, was a duly
appointed notary public as of the date of notarization of the afore-mentioned document on February
20, 2012, and was commissioned as such until December 31, 2013, and not until December 31, 2011,
as was inadvertently stamped on the Omnibus Sworn Statement dated February 20, 2012.
