This is to apply for a CONTRACTUAL APPOINTMENT under the Phil health Processing Unit of Quirino
Province Medical Center.

I was hired as a CASUAL status in the same hospital last January 16, 2014.

As a Phil health Processor of the said unit aside from the making copies, collates and routing the complete
documents of client’s PHIC claims, at the CF2 section upon our duty, we are obliged to consume between
8:00 am to 8:00 which has been already implemented last April 24, 2017 as per Governor’s recommended
for the patients has to settle beyond 5:00 PM.

I was really hoping that my contribution to ensure patient interest would enough in my application for the
contractual appointment.
