MEMORANDUM OF AGREEMENT

This Memorandum of Agreement is hereby entered into by and between:

The Sangguniang Barangay Council of __________________________,

represented by the Punong Barangay, Hon. _________________________________
with office address in ____________, Lezo, Aklan, herein referred to as THE BARANGAY
GOVERNMENT;

The _________________________________________________, represented by

___________________________________________, who is selling, dispensing and
retailing of commodities with an address in _____________________, Lezo, Aklan,
herein referred to as THE ESTABLISHMENT;

The ICS OPERATIONS OFFICER ON RELIEF OPERATIONS, represented by

_______________________________________, with office address in
__________________, Lezo, Aklan, herein referred to as THE RELIEF OPERATIONS
OFFICER.

WITNESSETH

WHEREAS, by virtue of the Republic Act 10121 also known as the “Philippine
Disaster Risk Reduction and Management Act of 2010, the Barangay Government of
______________________ provided with powers to effectively handle disaster
operations;

WHEREAS, the Local Disaster Risk Reduction and Management Council of the
Barangay Government of _____________________ has to Respond and manage the
adverse effects of emergencies, ensuring that there is an efficient mechanism for
immediate delivery of food to the evacuees and victims;

NOW, THEREFORE, in consideration of the above premises, the parties hereto

agree and bind themselves under the terms and conditions stated hereunder:

That during the term and continuance of this agreement, as herein defined, the
THE BARANGAY GOVERNMENT, THE ESTABLISHMENT and THE RELIEF OPERATIONS
OFFICER shall jointly-

 Plan, organize, conduct and manage the System for Relief Operations;
 Discharge and perform such other duties and responsibilities as may be
necessary for the fast, efficient and effective delivery of relief goods; and
 Help monitor the condition of the evacuees as well as their well-being.

THE BARANGAY GOVERNMENT shall-

 Allocate a Budget for Relief Goods and Operations;

 Together with the Relief Operations Officer and The Establishment, ensure
the accuracy of number of goods distributed and the costs of the relief
goods;
 Provide assistance needed for the Relief Operations;
 Organize Volunteers for the Relief Operations;
 Immediately pay the establishment for the goods acquired after the disaster;
and
 Submit report to the Provincial Disaster Risk Reduction and Management
Council.
 THE ESTABLISHMENT shall-

 Allow the Barangay Government to purchase goods with deferred payment

during disaster preparations;
 Actively participate in the relief operations;
 Ensure the accurate delivery of goods;
 Identify goods for relief operations; and
 Only give relief goods to identified relief operations personnel.

THE RELIEF OPERATIONS OFFICER shall-

 Identify its own personnel to withdraw the relief goods;

 Spearhead the relief operations;
 Receive and Distribute relief goods systematically; and
 Ensure all evacuees can avail the relief goods.

This agreement shall take effect on the date of its execution and shall remain in
full force during the entire period of the program implementation.
