JOINT AFFIDAVIT OF ACTUAL/ TRUE NAME

WE, P/SSUPT. BEN S. CASTILLA (RET.), of legal age, Filipino,

married, and resident of Tuazon Subd., Brgy. Lalo, Tayabas City and
MRS. LITA P. EBUNA, of legal age, City, after having been duly sworn
to in accordance with the law, hereby depose and state:

1. That, WE know personally and officially one ALDRIN M.

MALING a son of the spouses NARCISO V. MALING and
ERMINDA M. MALILANG;
2. That, he was born on 15 MARCH 1970 in Brgy., Puting-
Buhangin, Mogpog, Marinduque;
3. That, per Certificate of birth of ALDRIN M. MALING, his
mother’s name was in-advertently recorded as ERMINDA
MACUNAT instead of ERMINDA MALILANG;
4. That, per records of the NATIONAL STATISTICS OFFICE
which was issued a authenticated copy of Birth Certificate
indicated herein the correct spelling and true name of
ERMINDA MALILANG;
5. That, per Certificate of Baptism, MACUNAT is the surname
of ERMINDA’S mother, MARIA MACUNAT and MALILANG
is the surname of her father, SEVERINO MALILANG;
6. That, WE are executing this joint affidavit to prove and
attest that the true name of ALDRIN MALING’S mother is
ERMINDA MALILANG, and ERMINDA MACUNAT appearing
on the aforesaid Certificate of Birth refers to only one and the
same person, and that we are not in any way interested in
this affidavit except to attest and confirm the foregoing
statement of facts for all legal purposes;
