AFFIDAVIT OF VOLUNTARY SURRENDER

I, MRS. ERLINDA ABAYAN, of legal age, single, Filipino,

and resident of 429 M. Candelaria st., Maypajo, Caloocan City,
Metro Manila, after having been duly sworn to in accordance with
law, do hereby depose and say:

1. That I am the accused in the alleged offense of Grave

Threats in relation to child abuse with Criminal Case No.
68731, now pending before the Regional Trial Court,
Branch 124 of Caloocan City;

2. That the bail recommended for the provisional liberty is

in the amount of Two Thousand Pesos (P2,000.00)as
fixed by the Honorable Court;

3. That the accused hereby voluntarily submit himself on

the jurisdiction of the Honorable Court by filing the cash
bond in the amount of P2,000.00, thus for all legal
intents and purposes accused hereby manifest her
voluntary surrender hereof.

4. That I have executed this affidavit for the information of

the proper authorities, and for whatever legal purpose it
may serve.
