ACCOMPLISHMENT REPORT No. 11

Item Description

PREAMBLE
The unit rates "Total" for items in the bills shall be split into two sub-unit rates as follows:
(a) Unit rates for "Materials" shall include for the cost of materials (including all
wastage and for taxes, overheads and profit in connection with the ordering
of such materials) per unit of the items delivered to street level of site.
Sundry materials for fixing such as adhesives, mortar for bedding and
jointing, grouting materials, tying wire, bolts, nails, screws, ties and the like
are to be included in "Others".
(b) Unit rates for "Others" shall include for:-
(i) Labor and all costs in connection therewith
(ii) Taking delivery, unloading, storing, returning
packing handling, hoisting, lowering, etc. of materials
(iii) Fitting and fixing materials and goods in position including all sundry
materials for fixing such as adhesives, mortar for bedding and jointing,
grouting materials, tying wire, bolts, nails, screws, ties and the like
and for painting and finishing as specified.
(iv) Use of equipment, plant and tools
(v) All cutting
(vi) Establishment charges, overheads and profit
All prices shall be exclusive of 12% VAT.
The quantities given are indicative only and do not
represent a final bill of quantities.

PRELIMINARIES AND GENERAL REQUIREMENTS


Note : Contractor shall provide the detailed breakdown of costs in separate sheet.
Mobilization
Allow for mobilization of all necessary construction plant and
equipment, manpower and field facilities to site
Demobilization
Allow for demobilization of construction tools and equipment, manpower and field
facilities from site after completion of the work.
Temporary Facilities (as applicable)
This item represents provision, installation, maintenance and dismantling costs
of temporary facilities such as offices, warehouses, workshops, including
furniture, utilities, toilets and sanitary facilities, temporary access, fencing, etc.
Safety Requirements (as applicable)
Allow for the full compliance of the local and international safety, health &
environment provisions including all necessary and required protective equipment
for all workmen carrying out the works, safety sign boards, caution tapes, blue
sheets and all necessary protections to prevent access for others during
construction and after working hours.
Construction Equipment (as applicable)
Allow for the provision of necessary construction equipment including, but not limited
to, normal and special construction equipment and vehicles, instruments,
lifting devices, ventilating machines as required for the completion of the works.
Bonds & insurances, and warranties (as applicable)
Allow for the provision of all insurances, security and bank guarantees, and
warranties as required in the contract.
Permits and Licenses (as applicable)
Allow for the provision of necessary work permits and clearances and other permits
required for the completion of the work.
Drawings (as applicable)
Allow for the provision of all shop, as-built drawings and designs including all
necessary Professional Indemnity insurances.
Designs and PI Insurances (as applicable)
Allow for the provision of design services incuding all necessary
Professional Indemnity Insurances. ( Note: 5 Million Coverage)
Samples and Test Panels (as applicable)
Allow for the provision of samples and test panels as required by relevant
specifications for the proper execution of the works.
Brochures and Manuals (as applicable)
Allow for the provision of necessary manuals and brochures
for all but not limited to equipment supplied and installed.
Test and inspection (as applicable)
Allow for the provision of necessary site test and inspection works as required
by the relevant specification, in accordance with the test and inspection procedure,
using his own personnel and testing devices.
Security (as applicable)
Security of all materials, equipment, tools and all related works shall be included.
All works should be secured until completion of the works.
Scaffoldings (as applicable)
Allow for the provision of all scaffolding for the execution
and completion of the works.
Site Supervisory and Administrative Staff
Allow for the provision of adequate and competent site supervisory and
administrative staff for the proper execution of the works. Provide table of rates.
Taxes and duties (as applicable)
Allow for the provision of all applicable taxes and duties necessary for the
execution of the works except for the direct materials.
Disposal of Waste (as applicable)
Allow for the provision of disposal of all construction waste
and debris on the designated dumping area by LRMC.
Others (as applicable)
Other obligations not listed above but are contingently required for complying
with the requirements and provisions in the Contract.

Waste Water Collection System

Degreasing
Degreasing and removal of sludge shall perform as described regardless of the character
of sediments encountered, within the limits of work, to the lines and grade indicated
in the drawings & depths indicated, as specified.
