I, ____________________, of legal age, Filipino, married and a resident of

Comunal, Calapan City, Oriental Mindoro, after having been duly sworn to in
accordance with law, depose and state that:

1. I am the registered owner of a motor vehicle (TRICYCLE WITH SIDE CAR)

which is particularly described as follows:

Color : Blue

2. The original class of said motor vehicle was "PRIVATE";

3. I intend to change the use / class of said vehicle from “PRIVATE” to “FOR
HIRE”;

4. I have secured from the Tricycle Franchising Regulatory Board of the City
Government of Calapan herein attached Franchise with number 2181 for
the said purpose;

5. The change of class of said motor vehicle is not intended for any illegal or
unlawful purpose;

6. I am executing this Affidavit to attest to the truth of the foregoing and in

support of the application for the change of class of the above-described
motor vehicle on its Certificate of Registration.
